Qualifications:
An Associates Degree and 2 years experience in a related field or High School
Diploma and 4 years experience in a related field
Experience with persons with developmental disabilities or similar population
Experience with mental health/behavioral issues
Must be computer literate
Excellent verbal and written communication skills
Must possess a valid drivers license
Must have a minimum of one year supervisory experience
Ability to advocate for Consumer needs and desires
Be flexible and adaptable to change

Essential Duties:

To protect and promote the health, safety, and well being of Consumers.
Must visit sites during all three shifts on a monthly basis.
Manage ARC Recreational activities on rotating basis.
Provide on site supervision during recreational activities to ensure full participation
of consumers and staff. Document Consumer attendance.
Supervise/Monitor Consumer activity.
Do the daily program operations and scheduling to include the assigning of staff and
scheduling of Consumers in conjunction with the recreation department and
community activities.
Coordinate Consumer medical appointments with the Nursing staff to include
transport.
Report and submit a Behavior Referral of any unusual Consumer behavior to the
Behavioral Specialists.
Daily/weekly reports on the following issues will be required:
1) Medication Reports (Daily MAR Check, weekly pill count, medical
appointment forms and scripts)
2) Recreational Participation Levels
3) Food Purchases
4) Quality Assurance Reports(CIRS and Critical Log Reports)
5) Vehicle Logs
Develop schedules to ensure proper coverage per DDD Annex A and to
minimize/eliminate overtime.
Prepare daily assignments for all staff for all shifts, at least one week in advance (ie:
medication, staff duties and Consumer activities.
Must carry the MOD phone on a rotating basis to cover staff call outs and respond to
program emergencies within 15 minutes.
Operate a 12-passenger van
Supervise, orientate and train technicians.
Train staff on program orientation within one week of start date.
Perform staff Performance Evaluations
Maintain and monitor critical logbook and communication log.
Develop, implement and monitor the implementation of Consumers Individual
Habilitation Plans. Report IHP status and improvement plan to Deputy Director
quarterly.
Conduct monthly staff, Consumer meetings and quarterly Guardian meetings.
Develop open and professional communication between all staff members
Responsible for completing in a timely manner and appropriately maintaining
required reports/records to meet licensing, including:
Monthly staff and Consumer meeting minutes.
Program performance report/progress reports.
Consumer attendance report.
Daily IHP tracking.
Work Orders
Monthly safety drills, and monthly/quarterly inspections
Order, administer, document and store all consumer medications in accordance with
DDD regulations. Initial_____________

Responsible for petty cash for the residential facility. Must submit receipts to the
Director of Residential Services.
Review staff timesheets to ensure accuracy and send to Senior Managers of
Residential Services.
Report all incidents to supervisor and complete required documentation within one
business day, including but not limited to disciplinary actions, CIRS Forms, and
Employee Injury Forms.
Abide by DDD licensing regulations and ensure that the program upholds all
licensing standards
Cooperate with the agency and the DDD staff during any inspection or investigation
Document and report all employee injuries to the Human Resources Department.
Perform all other job-related duties as assigned by supervisor

Budget: Meet with Controller or designee on a monthly basis to review the
current status of their budget in areas that they can control; spending that is
associated with daily operations, i.e., utilities, overtime, program supplies,
food, leisure, household supplies, etc. Monthly budget reviews are to
determine budget status and document actions to correct any shortfalls.
Reasons for any shortfalls are to be documented, i.e., staff shortages, loss of
revenue, overtime, etc. Actions to remedy the reason for the shortfall are to
be documented, i.e., will hire staff by certain date, will reduce overtime by %
by certain date, etc. Actions to be taken to correct budget shortfall, i.e., staff
reduction, reduction in transportation costs, increased revenue, etc.
All efforts to remedy issues must be documented and addressed with
immediate supervisor and controller, such as the need to hire, order supplies
or coordinate with other departments.
Meetings: Attendance at meetings is mandatory, i.e., Budget, Human
Resources, Personnel, Managers meetings, and any other meeting that you are
required to attend and/or deemed necessary by your supervisor.
Staff Meetings: When conducting monthly staff meetings, the following
format is to be followed:
1. Agenda
2. Attendance Sheet
3. Minutes of the Meeting
Action Items need to be clearly identified

Schedule Overview:
Basic Schedule will be Monday Friday
Required to work one overnight and one weekend shift on a monthly basis.
