Dear Sir,
I, Govind Sendre S/o. Late Mr. Barati Sendre, residing at Ward No. 6, Katangi,
Dist Balaghat M.P. 481445, am a registered account holder of Sahara Super
Scheme as per the following details:

I declare and accept that, the above stated Shara Super Scheme D
Certifcate / Passbook of the Company has been lost and the same is not
traceable by me. I further declare that, I have not mortgaged, pledged to any
person the said Sahara Super D Certifcate / Passbook. I agree and promise
to return forthwith to the Company the said original Star Plus Passbook in
fund in future, for the cancellation.

I, the above named person, hereby state and confrm that, in consideration of
the company, issuing such duplicate certifcate of the Star Plus Passbook on
my request, I undertake to indemnify the company at all times and from time to
time save, defend and keep the company harmless including its successor(s),
assignee(s), and the director(s), Sahara holders, ofcers, workers etc. thereof
and respective heir(s), executor(s), administrator(s) each of their estates and
efects, harmless from and against all action, costs, suits, proceedings, claims
and demands whatsoever on account of the said Sahara D certifcate or any
of them, for loss or non production of certifcate(s) thereof on the part of any
person(s) whatsoever and against all damages, costs, expenses and sum of
money incurred in respect of or otherwise, for any misuse/ claims.
I, further declare and agree by these present that, in the event of my failure to
compensate or indemnify the cmpay in any action / suits and proceedings,
which the company may face for having issued the duplicate Sahara D at my
request, the company shall have lien on all the bonds which I am now holding
or I may hold thereafter including the interest accrued of payable on the said
bonds and other movable / immovable property owned / possessed singly /
jointly by me.
I, the surety, whose name and address are given hereunder promises and
undertake to return and redeliver such original pass book / bond of Sahra D
Bond / Pass Book release of duplicate Sahara D without such demand to
produce and return to the company, the original pass book / Certifcate when
found or traced and to take all corrective action, suits and proceedings at our
own cost as to defend and keep harmless the company for recovery thereof in
case the above named Pass book holder fails to comply with the above
statement. I further declare that I shall indemnify the company and its
successor(s), assignee(s), administrator(s) etc. against all the damages, costs,
charges, expenses etc. of whatsoever nature by the company in relation to
above Sahara D bond / Account Passbook.
If any dispute arises among the parties to this agreement, that dispute shall be
referred to sole arbitrator appointed by the company whose award shall be fnal
and binding on the parties. Venue of Arbitration shall be at Lucknow.
