Laparaskopi Nefrektomi pada Anak dengan non-fungsional Ginjal kiri

Pendahuluan : Operasi laparaskopi nefrektomi pertama kali dilaporkan oleh
Clayman pada tahun 1990. Di Indonesia sendiri, teknik laparaskopi nefrektomi
pada anak masih sangat jarang dilakukan. Ini adalah pengalaman pertama
laparoskopi nefrektomi di Rumah Sakit Hasan Sadikin, Bandung.
Tujuan : Melaporkan efektifitas dan keamanan laparaskopik nefrektomi pada
seorang pasien anak dengan non-fungsional ginjal kiri + stenosis ureterovesico
junction kiri + hidronephrosis kiri.
Laporan kasus : Seorang anak perempuan usia 4 tahun datang dengan keluhan
ISK berulang sejak 8 bulan. Pada pemeriksaan KUB-IVU kesan non-fungsional
ginjal kiri. Kami diagnosa pasien dengan non-fungsional ginjal kiri + stenosis
ureterovesico junction kiri + hidronephrosis kiri. Kami lakukan nefrektomi ginjal
kiri per laparaskopi.
Hasil : Operasi laparaskopi heminefrektomi berlangsung selama 2 jam 15 menit
dengan sukses tanpa konversi ke open. Pasien pulang pada hari ketiga setelah
operasi tanpa mengalami keluhan apapun juga.
Simpulan : Laparaskopi heminefrektomi efektif dan aman dilakukan pada anak
dengan non-fungsional ginjal kiri + stenosis ureterovesico junction kiri +
hidronephrosis kiri.

Laparoscopic Heminephrectomy in a child with Complete Double Collecting
Right Upper Moiety Kidney, VUR Grade V, and Suspected Neurogenic Bladder

I ntroduction: Laparoscopic heminephrectomy was firstly reported by Jordan and
Winslow in 1993. Laparoscopic heminephrectomy has recently been described
as a feasible procedure for the treatment of poorly functioning renal moiety in the
duplex kidney. However, this minimally invasive technique is a complex
procedure that has remained confined to the hands of experienced teams, with a
limited number of reports of relatively few cases. In indonesia, it has never been
reported before-
Objective: To report the effectivness and safety of laparoscopic heminephrectomy
in a child with complete double collecting right upper moiety kidney, VUR grade
v, and suspected neurogenic bladder.
Case report: A 1-year-old female child with chief complaint of straining
micturition since 4 months old. In non-contras MRI Urology, we found
complete double collecting right upper moiety kidney, vur grade v, and suspected
neurogenic bladder. In VCUG, We found VUR grade V and neurogenic bladder.
We diagnosed the patient with LUTS ec suspected neurogenic bladder and VUR
grade V with complete double collecting right upper moiety kidney. We performed
laparoscopic right heminephrectomy .
Result: Laparoscopic heminephrectomy was successfully done without conversion
to open for 2 hours and 40 minutes. Patient was discharged 3 days after
operation day without any complaints
Conclusion: Laparoscopic heminephrectomy was effective and safe to treat a
child with complete double collecting right upper moiety kidney, VUR grade V
and suspected neurogenic bladder
