1. ELIGIBILITY: Only open to licensed dentists who are residents of the 50 United States and the
District of Columbia (except residents of Puerto Rico and dentists licensed to practice in Illinois,
Massachusetts, Michigan, Minnesota, New Jersey, Puerto Rico, Rhode Island or Vermont) dentists
include general practitioners, oral surgeons, endodontists, prosthodontists, orthodontists, and
periodontists, who are at least 18 years of age or the age of majority in their state of residence, whichever
is greater at the time of entry. Void where prohibited by law. This promotion is governed by U.S. law and
is subject to all applicable federal, state and local laws and regulations. Employees of Henry Schein
Dental, a division of Henry Schein, Inc. (Sponsor), and its affiliates, subsidiaries, parents, advertising,
promotion and fulfillment agencies and members of the immediate family or household of each, are not
eligible to participate. Immediate family member is defined as a parent, sibling or any person residing in
same household as employee.

2. ENTRY: Sweepstakes begins on October 23
rd
, 2014 at 8:00 a.m. CST and ends on October
25

, 2014 at 5:00 p.m. CST. All entries become the property of the Sponsor and will not be returned. To

3. PRIZES/ODDS Three (3) $100 Henry Schein Dental Merchandise Certificates are available to be
won (one certificate per winner). Approximate retail value: $100.00 each. Certificate must be applied to
minmum order of $1,000 or more in Henry Schein Merchandise for redemption. The odds of winning
depend on the number of eligible entries received. ALL EXPENSES, TAXES AND COSTS
ASSOCIATED WITH ACCEPTANCE AND USE OF PRIZES NOT SET FORTH IN THESE RULES ARE
THE OBLIGATION OF THE WINNERS. Prizes are not transferable, except at discretion of Sponsor. Any
prize or prize portion not accepted, unclaimed or unused by a winner will be forfeited and will not be
substituted. This prize is non-transferable and no substitution is permitted, except by Sponsor, who
reserves the right, in its sole discretion, to substitute a prize of equal or greater value, if prize, or portion of
prize, becomes unavailable.

4. DRAWING: A random drawing will take place on or about October 30
th
, 2014 and the potential
winner will be selected from all eligible entries received. The potential winner will be notified in person, by
email, telephone, overnight carrier or regular mail within approximately ten (10) days of the drawing. If
Sponsor is unable to contact the potential winner after reasonable effort, or if the potential winner fails to
respond within three days of first attempted notification, prize may be forfeited and awarded to an
alternate winner.

5. GENERAL: The potential winners will be required to sign and return a Warranty of Eligibility and
Liability Release, and, where legal, a Publicity Release, within ten (10) days of first attempted notification.
In the event of noncompliance, the potential winner may be disqualified and an alternate winner selected,
at Sponsors sole discretion. By entering, participants (a) agree to be bound by the official rules and the
decisions of the Sponsor (including but not limited to eligibility determinations) which are final and legally
binding in all respects; (b) agree to release and hold harmless Sponsor, and its employees and agents
from any and all liability, loss, damage or injury resulting from participation in this Promotion or awarding,
receipt, possession use and/or misuse of any prize awarded herein and acknowledge that Sponsor and
its employees and agents have neither made nor are in any manner responsible or liable for any
warranty, representation, or guarantee, express or implied, in fact or in law, relative to any prize including,
but not limited to, its quality, mechanical condition or fitness for a particular purpose; and (c) consent to
use of his/her name, photograph and/or likeness for advertising and promotional purposes in connection
with this sweepstakes without additional compensation, restriction, notice, review or approval unless
prohibited by law. All entries are subject to Sponsors privacy policy (located at www.henryschein.com).
No responsibility is assumed by Sponsor for lost, late, misdirected, illegible, incomplete, stolen, or
mutilated entries or prize notifications.

6. WINNERS LIST: For the name and state of residence of the winners, send a self-addressed

7. SPONSOR: The sponsor of this promotion is Henry Schein Dental, a division of Henry Schein,

8. GOVERNING LAW/VENUE: Except where prohibited, all issues and questions concerning the
construction, validity, interpretation and enforceability of these Official Rules, or the rights and
obligations of entrant or Sponsor in connection with the sweepstakes, shall be resolved
individually, without resort to any form of class action, and shall be governed by, and construed in
accordance with, the laws of the State of Wisconsin, without giving effect to any choice of law or
conflict of law rules (whether of the State of Wisconsin, or any other jurisdiction), which would
cause the application of the laws of any jurisdiction other than the State of Wisconsin. In addition,
any dispute relating to the sweepstakes (including these Official Rules) shall be brought in the
appropriate state or federal court having jurisdiction over the subject matter, located in the State
of Wisconsin. Entrants hereby irrevocably consent to the personal jurisdiction of said courts and
waive any claim of forum non conveniens or lack of personal jurisdiction that they may have. Any
and all claims, judgment and awards shall be limited to actual out-of-pocket costs incurred, but in
no event attorneys' fees and under no circumstances will entrant be permitted to obtain awards
for, and entrant hereby waives, all rights to claim, indirect, punitive, incidental and consequential
damages and any other damages other than for actual out-of-pocket expenses, and any and all
rights to have damages multiplied or otherwise increased.

9. The federal government imposes certain restrictions on, and requires public reporting of, transfers
of value to a practitioner. Accordingly, for certain transfers of value we require your agreement and
certification that your practice is neither enrolled in nor bills any federal health care program (e.g.,
Medicare, Medicaid, Tricare, Childrens Health Insurance Program) for dental, oral surgery or
pharmaceutical services. Accordingly, you agree and certify that to your knowledge neither your practice
nor any of your patients are enrolled in or reimbursed by federal programs for such services, and that if at
any time such practice status changes, you will promptly notify us in writing.
