PATIENTS OUTCOME CRITERIA NURSING ORDERS

As soon as the patient is admitted and discharged


from the hospital. She and her significant others
will be able to have health care compliance and
competence.
1. ASSESSMENT
Assess patients level of
understanding.
Assess for signs and symptoms of
infection.



Monitor vital signs specifically
patients temperature for
occurrence of infection.
Monitor patients blood pressure.

2. PLANNING
Make a chart on medication
schedule.
Plan for a scheduled return visit
to the attending physicians clinic
follow-up check-up.

3. IMPLEMENTATION
Medication
Follow full course of drug therapy ordered by the
physician.
Environment
Maintain a safe environment, by keeping
surroundings clean.
Treatment
Comply with the medication regimen, follow-up
check-ups and dietary instructions.
Health Teaching

Perform daily personal hygiene
Discuss the proper way on how to
care for a surgical wound in the
home setting












Facilitates planning of discharge plan.

Encourage patient to report immediately to
health care provider if signs and symptoms of
infection is experienced, such as :
High fever
Redness on the incision site.
Teach patient and SO on how to take the
temperature of the patient.

Teach patient and SO on blood pressure
taking.

Encourage patient to note down the timing of
drugs.
Encourage patient to give importance to her
recovery and treatment.



Follow the course of therapy as prescribed.


Demonstrate techniques to reduce the risk of
infection.

Explain to the patient and to S.O the
importance of compliance of the medical
therapies.
Prior to discharge, it is necessary to check any
medical instructions for home care.
Discuss to the patient and the S.O all the
necessary details needed upon doing proper
incision and dressing care.
Demonstrate to the S.O the following
steps as follows.
Change your wound covering if blood
starts seeping through it. Apply pressure
to the wound for a few minutes to try
and stop bleeding
Avoid heavy lifting
Have the S.O conduct the wound








Out-Patient Referral
Seek for medical help for any unusual changes.
Diet
Appreciate the importance of
having a balanced diet and
provide a menu for optimal
nutrition.
Ensure iron intake by eating iron
rich foods such as: beef, canned
tuna, and chicken.
Encourage client to take in iron
with proper amount of vitamin C.
Encourage low salt and low fat
diet.
Spiritual - practice together with her
family a strong spiritual health.


EVALUATION
Evaluate patients understanding of health
teachings provided.

Evaluate self-compliance of all treatment
regimens.

Evaluate progress of ones health
condition.

breathing exercise because patient is
often reluctant to breathe deeply for
fear of any pain caused by the incision
Instruct patient to notify physician if unusual
changers occurs.

Explain to the patient the importance of iron
intake.

Explain to the patient how vitamin C affects
iron absorption.
Discuss to the patient how low salt and low
fat diet affects blood pressure.
The practice of religious faith is hence
encouraged within acceptable limits.
Allowing the patient to conduct some time
for religious reflection.

Evaluate patients understanding on the
health teachings given by asking follow- up
questions.
Discuss to the patient and the S.O. the
importance of evaluating self-compliance in
order to monitor the patients condition.
Encourage the patient and the S.O. to
evaluate any progress on the patients
health.
