UNIVERSITY OF CALIFORNIA, BERKELEY
DeCal: PreMed 101

APPLICATION FOR: Course Facilitator
DUE DATE: Wednesday, November 9th - 6pm via HARDCOPY

Dear Applicant,

Thank you for your interest in joining the PreMed 101 team. Our mission is to offer
comprehensive, reliable information about the medical school application process & the field of
medicine at large for all students at UC Berkeley. PreMed 101 is a unique premedical entity on
the UC Berkeley campus; it does not require any sort of application process in order for students
to access the services & resources that the course offers. We, the Facilitators of PreMed 101,
strongly believe that all premedical students should be judged on their merits alone & afforded
the same opportunity when pursuing the MD & should not be hampered by a lack of reliable
information.

Our team prides itself on its autonomy; the entire course is operated with minimal involvement
by faculty. Decisions are reached by consensus, problems are dealt with within the team, & each
member is afforded the respect he or she deserves. As such, group dynamics are of utmost
importance. This application will hopefully indicate a little bit about your character, motivations,
& potential ability. Once you have returned the completed application, you may be invited to an
interview. If selected after the interview process, you will then shadow our Facilitator team for
several weeks to give us a better idea of how you will work in our group and to give you a better
idea what exactly a Facilitator does.

We are searching for 2-3 Facilitators to replace our graduating team members. If you have any
questions, please ask any one of us as wed be more than happy to answer. The more you know
about the position, the better prepared you will be for the interview.

You will hear back from the team if you have moved on to the interview round within a week of
the submission deadline. Please refer to the timeline at the end of this application.

GOOD LUCK and GO BEARS!
