sagissait avant tout dtablir le risque court terme et de
choisir un traitement en fonction de ce risque. Vous arri-
vez aisment dlimiter trois groupes de patients :
i possibilit de SCA;
i risque faible ;
i risque intermdiaire ou lev (o lapproche est assez
similaire, sauf pour ce qui est de lusage des AGP2b3a et
du clopidogrel).
Possibilit de SCA
Lincertitude diagnostique implique ici labsence de tout
critre clinique, lectrocardiographique ou biochimique de
risque plus lev, associe habituellement un risque faible
de maladie coronarienne sous-jacente. Dans ce contexte,
lAAS peut tre sufsant.
Lvaluation pourra comprendre, si cela est indiqu, la
surveillance clinique et le monitorage, des ECG sris et un
dosage rpt des marqueurs. Cette dmarche dvaluation
permettra :
i de dceler des critres de risque plus lev absents
lvaluation initiale ;
i de procder un test de stratication appropri, si cela
est indiqu (preuve deffort, scintigraphie myocardique
ou chographie deffort selon la disponibilit), essentielle-
ment an de faciliter ltablissement dun pronostic plus
long terme.
Risque faible
Pour les patients souffrant probablement dun SCA mais
demeurant risque faible, lAAS et un BB sufsent habi-
tuellement. Au terme dune priode dobservation, le pa-
tient qui ne prsente pas de changements lectrocardio-
graphiques ou biochimiques (marqueurs) pourra subir un
test de stratication, essentiellement pour dceler sil souffre
dune maladie coronarienne plus grave et tablir un pronos-
tic plus long terme, ce qui permettra de moduler lap-
proche et le traitement en fonction du risque. Si le rsultat
du test est normal ou que le patient est faible risque, on
pourrait lui donner son cong aprs lui avoir prodigu les
conseils dusage, pris les mesures ncessaires pour quil ait un
suivi externe et avoir amorc une thrapie anti-angineuse
approprie.
Risque intermdiaire ou lev
Le traitement associera une thrapie anti-ischmique
adquate au traitement antithrombotique appropri : BB,
nitrates, AAS et hparine. Le clopidogrel pourra tre pres-
crit aux patients exposs un risque intermdiaire, sur-
tout si on choisit une stratgie conservatrice. Les patients
prsentant un risque lev bncieront dun AGP2b3a et
dune orientation pour angioplastie.
Tous ces patients seront hospitaliss pour permettre
dtablir une stratgie conservatrice ou effractive, cest-
-dire oriente demble vers une angioplastie ou bien vers
un test de stratication aprs une phase de stabilisation.
Les patients rfractaires au traitement sont les plus
risque. La stratgie doit toujours tre nergique. Une an-
gioplastie rapide doit tre considre. Le traitement sera
maximal et inclura un AGP2b3a. Lusage du clopidogrel est
plus controvers. Un ballonnet intra-aortique pourra ser-
vir de pont jusqu langioplastie.

OTRE RSIDENT ayant trouv remarquable cette discus-
sion sur le traitement du SCA, il dcide de vous im-
pressionner et vous ramne le lendemain un magnique
schma rsumant la discussion. Ce schma trne dorna-
vant dans la salle de choc de lurgence, et vous vous plai-
sez rappeler, chaque fois que vous en avez loccasion, que
cest une discussion avec vous, un vendredi soir de fvrier,
qui a inspir cette magnique synthse. La gure rsume
donc lapproche globale du patient atteint dun SCA.
Une semaine plus tard, M. Essha, en pleine forme, vient
vous remettre une bouteille de Chteau dYquem 1990. En
le remerciant, vous en protez pour linformer que le vin
est dot de proprits anti-oxydantes, qui ont un effet sta-
bilisateur sur la plaque coronarienne, et quen boire per-
met donc dviter bien des dboires. M. Essha rit de bon

lurgence avec vous. c

Acute coronary syndrome in the ER: present norms. Acute
coronary syndrome treatment has dramatically changed in
the last few years. A systematic approach and a good under-
standing of the physiology of unstable angina is now essen-
tial to make optimal and well-suited therapeutic choices for
a specic patient. Risk stratication should rst help choose
a treatment level. The treatment itself can be divided into
three steps: reversing the ischemic process, preventing throm-
bosis and choosing the best approach: conservative or inva-
sive. These choices should always be linked to the clinical pre-
sentation. Basic pharmacotherapy should be well understood
and used. New treatments should be reserved for high-risk
presentations. A rapid angioplasty can also be helpful for this
category of patients. Recent literature should be critically eval-
uated for a better understanding of actual controversies.
