DIRECTIONS:
This questionnaire consists of four parts regarding employee engagement levels in the
Organization.
The first three parts are statements that are to be rated on a 5 point scale with options
ranging from Strongly Agree to Strongly Disagree. Please tick the option that is closest

The last part is a set of 5 open ended questions; please answer them in a sentence or
