furosemide
heart failure,
hepatic
Brand Name
impairment or
Lasix
renal disease.
Hypertension.
Classification
Diuretics

Action
Inhibits the
reabsorption of
food sodium
and chloride
from the loop
of Henle and
distal renal
tubule.
Increases renal
excretion of
water, sodium,
chloride,
magnesium,
potassium, and
calcium.
Effectiveness
persists in
impaired renal
function.

Contraindication
Hypersensitivity;
cross-sensitivity
with thiazides and
sulfonamides may
occur, Hepatic
coma or anuria;
Some liquid
products may
contain alcohol,
avoid in patients
with alcohol
intolerance.

Adverse Effect
CNS: blurred vision,
dizziness, headache,
vertigo
EENT: hearing loss.
Tinnitus
CV: Hypotension
GI: anorexia,
constipation, diarrhea,
dry mouth, dyspepsia,
nausea, pancreatitis,
vomiting
GU: excessive urination
Derm: photosensitivity,
pruritis, rash
Endo: hyperglycemia,
hyperurecemia
F and E: dehydration,
hypocalcemia,
hypochloremia,
hypokalemia

Nursing Responsibilities
>Assess fluid status. Monitor
daily weight, intake and output
ratios, amount and location of
edema, lung sounds, skin
turgor, and mucous membranes
>monitor blood pressure and
pulse before and during
administration.
>If administering twice daily,
give last dose no more than
5pm to minimize disruption of
sleep cycle
>IV route is preferred over IM
route for parenteral
administration
>PO: may be taken with food or
milk to minimize gastric
irritation. Tablets may be
crushed if patient has difficulty
of swallowing.

DRUG STUDY

Generic Name
ciprofloxacin
Brand Name
ApoCiprofloxacin,
Cipro, Cipro XR,
Proquin-XR
Classification
Anti- infectives

Indication
Treatment of
bacterial
infection
including Intraabdominal
infections

Action
Inhibit bacterial
DNA synthesis
by inhibiting
DNA gyrase.

Contraindication
Hypersensitivity.
Cross-sensitivity
among agents
within class may
occur.

Adverse Effect
CNS: seizures,
dizziness, headache,
insomnia, acute
psychoses, agitation,
confusion, drowsiness
GI: hepatoxicity,
diarrhea, nausea,
abdominal pain,
increased liver function
tests, vomiting
Derm: photosensitivity,
rash
Endo: hyperglycemia,
hypoglycemia
Local: phlebitis at IV
site
MS: tendinitis, tendon
rupture
Misc: hypersensitivity
reactions including
anaphylaxis.

Nursing Responsibilities
>Obtain specimens for culture
and sensitivity before initiating
therapy.
>If gastric irritation occur,
ciprofloxacin may be
administered with meals
>Ciprofloxacin 5% and 10%
oral suspension should not be
administered through a feeding
tube. Shake solution for 15
seconds prior to administration.

NURSING CARE PLAN

Assessment
Subjective: hindi
ako madalas
umihi kasi
pagtayo pa lang
nahihirapan na
ako as verbalized
by the patient.
Objective:
RR- 32cpm
Orthopnea
Presence of
edema in the
lower extremities

Diagnosis
Fluid Volume
excess related to
reduced
glomerular
filtration rate,
increased
antidiuretic
hormone
production and
sodium water
retention as
evidence by
orthopnea,
hypertension and
edema.

Planning
Within 6 hours of
nursing
intervention the
patient will be
able to
demonstrate
stabilize fluid
volume with
balance intake
and output, stable
weight, and
absence of
edema.

Intervention
>Assess for
distended neck
and peripheral
vessels. Inspect
dependent body
areas for edema.

Rationale
-Excessive fluid
retention may be
manifested by
venous
engorgement and
edema formation.

>low salt and low

fat diet

-To prevent
further edema

>measure
abdominal girth
for changes

-may indicate
increasing fluid
retention/ edema

>weigh daily

-provide a
comparative
baseline and
evaluates
improvement

>Elevate the
edematous
extremities

-to prevent further

edema

Evaluation
After 6 hours of
nursing
intervention the
patient was
demonstrate
stabilized fluid
volume with
balanced intake
and output, stable
weight and
absence of
edema.

NURSING CARE PLAN

Assessment
Subjective: hindi
ako makagalaw ng
maayos kasi kapag
gumagalaw ako
nahihirapan okong
huminga as
verbalized vy the
patient.
Objective:
RR- 32cpm
Positive signs of
fatigue, bed rest
and dyspnea

Diagnosis
Activity
intolerance is
related to
generalized
weakness,
prolonged bed
rest, fatigue and
dyspnea

Planning

Intervention

Rationale

Evaluation

Within 7 hours of
nursing
intervention the
patient will
achieve
measurable
increase in activity
tolerance,
evidenced by
reduce fatigue
and weakness and
by vital signs
within acceptable
limits during
activity.

>Provide
assistance with
self-care activities
as indicated.
Intersperse
activity periods
with rest periods.

-Meets patients
personal care
needs without
undue myocardial
stress and
excessive oxygen
demand.

>ascertain ability
to stand and
move about, and
degree of
assistance
necessary or use
of equipment

-to determine
current status and
needs associated
with participation
in needed desired
activities

After 7 hours of
nursing
intervention the
patient was
achieved
measurable
increased in
activity tolerance,
evidenced by
reduced fatigue
and weakness and
by vital signs
within acceptable
limits during
activity.
