PETITION
COMES NOW, the petitioner, PEBBLES B. PAREDES to this
honorable court, most respectfully states that;
1. Petitioner is of legal age, born on December 18, 1988,
single, Filipino citizen and a resident of Brgy. Dalakit,
Catarman Northern Samar;
2. Petitioners telephone number is 987-098-55.
3. Petitioner has completed her law studies at University of
Eastern Philippines;
4. Petitioner is a member of the Philippine Bar having been
admitted to the practice of law on March 30, 2014 as
evidenced by a photocopy of her Certificate of Membership
attached hereto and made an integral part hereof as Annex
A;
5. Petitioner is a member of good standing of the Philippine
Bar as shown by the following certification:
a. Certification of no pending Administrative case
issued by the Bar confidants Office of the Supreme Court,
Manila (Annex B);
b. Certification of no criminal case filed against her
issued by the Clerk of Court, Regional Trial Court, Catarman,
N. Samar (Annex C);

c. Certification as a member of good standing issued

by the IBP-Northern Samar Chapter (Annex D);
d. Certification of good moral character issued the
executive officers of the IBP-Northern Samar Chapter (Annex
E);
6. Petitioner has completed the required units in compliance
with the Mandatory Continuing Legal Education as mandated
by the Supreme Court. Attached herewith is a copy of her
MCLE Compliance Certificate (Annex F);
7. Petitioner has paid his membership fee with the Integrated
Bar of the
Philippines as per IBP Official Receipt No.
9101112, dated April 10 , 2014, a photocopy of which is
attached hereto as Annex I. He has also paid the
Professional Tax Receipt due as evidenced by PTR No.
12345678, dated June 20, 2014, a photocopy of which is also
attached herewith as Annex J.
8. Petitioner is not a member of any subversive association or
any association advocating to overthrow the existing
government;
9. Petitioner is not a government employee nor has he applied
for the same
commission in any other jurisdiction.

PRAYER
WHEREFORE, in view of the foregoing, it is respectfully prayed
to this Honorable Court after due notice, publication and hearing
to grant this Petition for Appointment of Notarial Commission of
the herein Petitioner for and in the Province of Catarman,
Northern Samar.

based on my personal knowledge or based on authentic
records.
4. That I hereby certify under oath in this petition as follows:
a. I have not theretofore commenced any action or filed any
action or claim involving the same issue in any court,
tribunal or quasi-judicial agency and, to the best of my
knowledge, no such other pending action or claim is
pending therein;

b. If there is such other pending action or claim, a complete

statement of the present status thereof; and
c. If I should thereafter learn that the same or similar action
or claim has been filed or is pending, I shall report that
fact within five (5) days therefrom to the court wherein
the aforesaid complaint or initiatory pleading has been
filed.
