CHECKLIST OF TECHNICAL REQUIREMENTS FOR BIDDERS

The first envelope shall contain the following technical information/ documents:

Eligibility requirements under Section 23.1 of Revised IRR of RA 9184

Class A Documents
Department of Trade and Industry (DTI) business name registration or SEC
Registration Certificate. Whichever may be appropriate under existing laws of
the Philippines supported with the necessary information using the prescribed
forms.
Valid and current Mayors permit/ municipal license.
Statement of prospective bidder of all its ongoing and similar completed
government and private contracts, including contract awarded but not yet
started. If any. Whether similar or not similar in nature and complexity to the
contract to be bid, within the relevant period as provided in the bidding
documents.
Valid PCAB license and registration for the type and cost of the contract to be
bid.
Audited financial statements, stamped received by the BIR or its duly
accredited and authorized institutions, for the immediately preceding calendar
year, showing among others the total assets and liabilities.
Computations of Net Financial Contracting Capacity (NFCC), OR Certificate
of commitment specific to the contract to be bid. Issued by a University or
Commercial Bank to extend to the bidder a credit line of awarded the
contract.
Class B Documents
If applicable, valid Joint Venture Agreement (JVA) or, in lieu thereof. Duly
notarized statements from all the potential joint venture partners starting that
they will enter into and abide by the provisions of the JVA in the instance that
the bid is successful shall be included in the bid.

Bid security in the prescribed form, amount and validity period

Project requirements, which shall include the following
(1.) Organizational chart for the contract to be bid
(2.) List of contractors personnel (project manager, project engineers, materials
engineers and foremen). To be assigned to the contract to be bid. With their
completed qualification and experience data.

iv.)

(3.) List of contractors equipment units. Which are owned, Leased, and/ or under
purchased agreements , supported by certification of availability of equipment
from the equipment lessor/ vendor for the duration of the project .
Sworn statement in accordance with Section 25.2 (b)(iv) of the IRR of RA 9184
and using the form prescribed (Omnibus sworn statement)

The second envelope shall contain the following financial documents:
Financial Bid Form
Duly Signed Bid prices in the Bill of Quantities
Duly Signed Detailed Estimates
Duly Signed Cash Flow by Quarter and Payments Schedule
