The competition is open to all Filipino families currently residing in the Philippines whose
family members are all of good moral standing and with no criminal record. Nominated families
should have at the minimum one child and have an advocacy that most, or if possible, all family
members actively participate in.
Anyone may nominate a Filipino family whom they admire, including their own families,
using the official Jollibee Family Values Awards entry form and must strictly comply with
competition rules and regulations. Entry forms will be available for free at Jollibee branches
nationwide, may be downloaded from and/or accomplished on Jollibees official website
atwww.jollibee.com.ph/familyvaluesawards.
Nomination forms should be filled out completely, providing full name, contact details and
address of the nominator and/or nominated families and answers to the questionnaire that establish
the nominees suitability and eligibility. Nominators may use extra sheets of paper if needed.
Nominations may be submitted on the merit of the following criteria:
30% Scope of the familys contribution to the community or institution they are assisting
30% Relevance of advocacy in addressing the needs of the community and sustainability of their
efforts
20% Exemplifies Filipino family values (e.g. generosity, compassion, creativity, resourcefulness, love
for the community, love for environment, strong ties)
10% Degree of participation of each family member in the project/s
10% Ability to surpass challenges and special circumstances in carrying out advocacies
100% TOTAL
Any nomination/entry found to have violated the competition rules & regulations will be
disqualified from the competition. Jollibee and its agency reserve the right not to award, and to
withdraw and/or forfeit any of the prizes should the participants or entries fail to meet a certain
competition requirement.
Deadline for the submission of all nominations/entry forms is on July 31, 2014, 11:59 PM.
All nomination/entry forms complete with the following details and photo documentations
should be placed inside an envelope and may be submitted by means of:

C. Online Submission
Participants/Nominators must access the online nomination form
at www.jollibee.com.ph/familyvaluesawards.
On the website, participants must click on the "Submit Nomination" Form and
completely supply all the necessary details and answer each question to the best of their abilities.
Once all the questions have been answered, participants must click submit and a
window with a verification will appear to confirm that the nomination form has successfully been
accomplished.
Participants will then be notified to check their inbox for a verification email which
will require them to finalize their nomination form by clicking on a link.
Upon clicking on the link, Jollibee will be notified that an entry has already been
submitted.
The completed nomination form will then be automatically forwarded to Jollibee
Marketing for screening.
D. For nominations sent via e-mail:
Participants/Nominators must download the digital nomination form
at www.jollibee.com.ph/familyvaluesawards.
Participants must completely supply all the necessary details and answer each
question to the best of their abilities and click on save.
Once all the questions have been answered, participants must attach the soft copy of
the filled out nomination form and submit to JFVA@jollibee.com.ph. An email verification will be
sent to confirm that the nomination form has successfully been submitted.
The completed nomination form will then be automatically forwarded to Jollibee
Marketing for screening.
For accompanying photos:
a. Sent via snail mail, personal delivery to Jollibee stores:

An individual or organization may nominate more than one family for the search. Nominees
who have previously won any national family-oriented awards are not eligible to join.
Families who previously joined the JFVA may still be nominated. However, the five families
who won in the previous JFVA will no longer be eligible to join.
All screening of entries and determination of the fifteen (15) semifinalists will be done with
DTI Representative and the screening committee composed of representatives from different
Jollibee groups (e.g. HR, Jollibee Foundation and Marketing group).
A total of fifteen (15) semi-finalists will be selected:
a. 10 Filipino families based in the Philippines
b. 5 OFW families based in the Philippines and/or abroad
The fifteen (15) semi-finalists shall be determined on August 23, 2014, from 9AM-12NN at
the 10th Floor, Jollibee Plaza, Emerald Ave., Ortigas Center, Pasig City.
All semi-finalists are required to go through an interview and submit supporting documents
such as: Marriage Certificate, Birth Certificate, Family Photos, NBI Clearance, Barangay Clearance,
and Letter of Recommendation from barangay captain or village president.

For families based in the Philippines: phone and face-to-face in-depth interviews.
o
For OFW families: phone calls to members abroad and in the Philippines, video (i.e.
Skype) and email interviews.
The top fifteen (15) will be judged by the panel of judges consisting of three (3) Jollibee officials and
six (6) well-known family personalities on August 29, 2014, from 2PM-5:30PM at the Crowne Plaza
Manila Galleria.

The six (6) families (five (5) families in the Philippines and one (1) OFW family) with
the highest scores from the tabulated score of the board of judges on August 29, 2014 will be
declared as the winners of the search. In case of a tie, the Chairperson of the Board of Judges will
determine the winner.
Apart from the six (6) grand winners of the 4th JFVA, the Board of Judges will also
select 5 families to receive special awards namely
a. Special Citation: Environment - Honorable families who lead their communities in protecting
and preserving the environment
b. Special Citation: Education- Outstanding families who are advocates of an individual or
communitys right to education
c. Special Citation: Specific Stakeholders (Children) - Families whose advocacies further the
welfare of particular groups
d. Special Citation: Specific Stakeholders (PWDs) - Families whose advocacies further the
welfare of particular groups
e. Special Citation: Specific Stakeholders (Women) - Families whose advocacies further the
welfare of particular groups
All finalists will also be required to read and sign the Terms and Conditions for
Acceptance of final nominations (see attached terms & condition in a separate sheet).

Stratworks Inc. (SWI) will collect all entries submitted via snail mail, JFVA microsite
(www.jollibee.com.ph/familyvaluesawards) and through Jollibee stores.
o
SWI will collate all entries received via snail mail and through the microsite in a CD
for safekeeping. Meanwhile, entries sent via snail mail will be date and time-stamped and
subsequently placed in sealed envelopes.
o
Jollibee stores must turn over all entries to SWI for safekeeping and for monitoring.
o
All entries received will be placed inside a secured cabinet during the nomination
period.
o
SWI will provide DTI a copy of all submitted entries.
b. The Jollibee screening committee will deliberate and determine the fifteen (15) semifinalists from
the following areas: Metro/Mega Manila, North Luzon, South Luzon, Visayas and Mindanao, as well
as other countries (for the special OFW category).
c. The fifteen (15) semifinalists will be subjected to on-ground validation and extensive background
research by a third party research agency to be commissioned by SWI and/or Jollibee.

d. After the research and verification process, Stratworks and Jollibee will submit the official list of
fifteen (15) semi-finalists to DTI.
e. During the final deliberation of the Board of Judges, representatives from the research firm will also
be present to respond to questions or clarify issues that may be raised by the judges regarding the
semifinalists background or their activities.
f. A DTI representative will be invited to be present during the determination of the six grand winners.
g. All winners will likewise be subjected to a final validation process/background check by Jollibee
Foods Corporation representative, and SWI.
h. Incomplete information or failure to provide all pertinent requirements in the entries will be
considered invalid.
i. Winners will be announced via an ad placement of winning families and will also be notified by
Telephone, e-mail and registered mail.
