Good day!
The College of Nursing aims to develop the leadership and management skills of the students
specifically the level IV BS Nursing students through Intensive Nursing Practicum which will
enable them to diagnose problems within the school, plan for different programs, implement the
programs related to each problem that was identified and evaluate the effectiveness of each
program as part of the School Nursing. One of the problems we have identified is the

In lieu with this, we would like to ask for your permission to have a softcopy of the students of
this university as part of initiating an electronic filing system in the infirmary of this university.
Aside from that, we would also like to conduct a mass on December 7, 2014 at the Iglesia
Filipina Independente, San Juan I, Ternate, Cavite. This endeavors the residents of San Juan I to
become united as one in terms of Spirituality with you as the priest to give the inspirational
message for everyone.`

PROPOSAL LETTER
Title of Project: Election of Officers in each Organization; Holy Mass
Theme: ISANG MISA GABAY TUNGO SA PAGKAKAISA.
Proponents: Level IV BS Nursing Students
Rationale:
The level IV BS Nursing students aim to provide holistic approach in every problem their
clienteles have perceived. One of this is lack of activities related to Spirituality. One of the main
objectives of the class is to initiate activities that will facilitate promotion of welfare and
development of the community people in different aspects of life including Spirituality.
Objectives:
As the residents continue their everyday lives in the community, keeping in touch with
the Almighty is very essential. Lack of involvement to church activities is a factor in which
people tend to have an impaired relationship to God and engage with worldly activities. The
following are the objectives of the level IV BS Nursing students in pursuing this project:

To strengthen the relationship with the Almighty.

To have elected officers in existing organizations linked to church to commence
religious activities in Barangay San Juan I, Ternate, Cavite.
To unite the community people through conducting a mass to serve as inspiration to
everyone.

Date and venue:

The election of officers for the existing organizations will be on December 3, 2014 beside
the Iglesia Filipina Independente Church (Aglipay Church). The mass will be held in the Iglesia
Filipina Independente Church on December 7, 2014.

Mechanics:
Post advertisements about the upcoming election of officers for Wopic, Kalifi and Yifi
Organizations.

Invite each family in Barangay San Juan I (F. Del Rosario St. and Narvaez St.)
through home visits.
Divide the attendees into three (3); men, women and youth and conduct the election
for each organization supervised by the President of the Parish Council or the Youth
Representative.
Plan programs to which the officers will give suggestions that will help the
community people to strengthen their relationship with the Lord.
Let the officers know about the plan of having a mass.

Strategic Plan:
To be reviewed by the level IV Clinical Instructors.
To be approved by the Parish Council.
To be conducted by the BS Nursing Students in cooperation with the Wopic, Kalifi
and Yifi Organizations.
Participants:
The participants for the said activities will be the residents of Brgy. San Juan I
specifically the families from F. Del Rosario St. and Narvaez St. Other members of the
community are also welcome to join.
Budgetary Estimation:
Budget estimation will be reviewed by the whole class together with the clinical
instructors.
