Add Application form: Solo essential
Add Plan: Solo essential
Add coverages (refer to the attached template)
Replicate the following screens : Create case (Generic Field appendix),
Medical info, Doctor info and Non medical info . Link them to the new
application form.

Add specific plan name: Solo essential
Add coverages (refer to the attached template)
Adjust the occupational category and the waiting period values.

3. Add values 1,2,3,4,5,5B and 6 in the dropdown of occupation class


(Decision tool screen) (refer to Miscellaneous template).
4. Add the not applicable value in the dropdown of the status (Insured
screen) (refer to the Field appendix template).

Notes:
1. Estimate or request for clarification will be provided within 5 days of receipt
of the CCP form.
2. CCP will be considered active after approved by an authorized client
representative.
3. Delivery date will be communicated after approval of CCP.
4. All work completed will require acceptance from client. Acceptance is
expected within 24-48 hours of work completion.
5. Changes to the scope of this CCP will require a revision to be issued and
may affect price and duration estimates
6. Price and duration estimates are based on description of requested change
as outlined in this CCP.
7. Cost and time estimates may vary by 20% depending on final approved
requirements and detail design.
8. Final Cost will be billed on an actual basis using current rates.
9. An invoice will be submitted to client for payment within 5 days of work
completion. If the CCP is cancelled or put on hold, all work completed to
that point would be prorated and submitted to the client for payment.
