USER ACCEPTANCE TEST FORM

Purpose of Testing

Form-Based Testing: ensure that individual forms are performing

correctly, required fields and buttons exist, and the flow of
information and data entry is logical and correct based on the
business requirements.
Business Process Testing: ensure that the prioritized business
functions and processes are supported by the application, flow of
data and screens is logical for each business process, and security
and access requirements are met

I. PATIENT LOG-IN
This page shows the log-in page of the
patient where in there is username and
password they to fill up so that they can
login.
By clicking log-in button patient can view
his/her page or the patient page.
An error message is displayed if required
fields not filled out.

wrong username and password.
By clicking dashboard button patient can
view
the
Medical
Profile
button,
Appointments button, Doctor List button
and Log-Out button.
By clicking Medical profile button, displays
all the Medical Record of the patient the
date, time, findings and the doctors name.

latest and past appointments they do and
also the status of the appointment they
asked.
By clicking Medical kit () button w/c is the
new

II. DOCTOR LOG-IN
By clicking log-in button doctor can view the
doctor page that has consultations,
appointments and schedules. All this they
can view if the username and password will
match.
By clicking dashboard button doctor can
view the Consultations, Appointments,
Schedules and Log-Out.
By clicking consultations button doctor can
view all the patients who asked for
consultation to them.
By clicking magnifying glass () button in the
consultations, the doctor can view the
medical record of the patient that they
consult.
By clicking medical kit box() button in the
consultations,
By clicking appointments button the doctor
can view who are requested for
appointment to them.
By clicking check () button in the
appointments, the request appointment of
the patient will automatically change the
status into accept.
By clicking x button in the appointments,
the request appointment of the patient will
automatically change the status into reject.

NURSE LOG-IN
This page shows the log-in page of the
Nurse where in there is username and
password to fill up so that they can login

and view the billing and maintenance.

By clicking log-in button Nurse can view
his/her page or the Nurse page.
An error message is displayed if required
fields not filled out.
An error message is displayed for the
wrong username and password.
By clicking the add () button under patient
page it can add or register a new patient.
By clicking the Edit () button under patient
page it can update patient present profile
By clicking Billing () button the nurse can
view the billings / bills of the current patient.
Here is where the nurse compute all the
bills of the patient including discount if
senior citizen.
By clicking New button () first it will create
new entry on billing database and also to
activate the text boxes in billing page.
By clicking the patient drop box () under
billing page it will release the current or the
registered patient at that date.
By clicking next drop box () under billing
page the nurse can select clinic name.
By clicking medicine drop box () under
billing page the nurse can select different
medicine present to that clinic.
By clicking Price text box () under billing
page the nurse can input no items or no. of
medicine.
By clicking Add button () under billing page
the nurse can view bellow is the name of
the medicine, price and the quantity.
By clicking Add Discount button () under
billing page the nurse can add discount if
senior citizen.
By clicking Add button () under billing page
the system will automatically save the data
into the database and pop-up receipt will
appear.
By clicking Add button () under billing page
the nurse can print the receipt.
By clicking medicine button () the nurse can
view the medicine names, description, unit
and quantity. The nurse can also view the

maintenance of the medicine assigned on

that clinic. Here is where the nurse update
the medicine whether it is still available or
not and also add medicine if possible.
By clicking the Add () button under
maintenance page the nurse can add or
register a new medicine.
By clicking the Edit () button under
maintenance page the nurse can update or
edit the current medicine present on that
clinic.
By clicking the Delete () button under
maintenance page the nurse can delete or
erase certain medicine if it is obsolete
already.
