against terrorism and organized crime when financial
transactions take place between the USA and EU
states and countries outside the US / EU. You have
been directed to this form as you are required to
submit a full set of personal identification details in
order to participate in a financial transaction. Failure
to complete this form fully and accurately will result
in the denial of your participation in the transaction.
What you have to do
 Complete all sections of the form in BLACK ink
 Photographs must be in full colour. Digital photo-
graphs are acceptable.
 Finger, foot & nose prints must be clear and distinct
and in BLACK or BLUE ink
 Make no marks in any section labeled “For Official
Use Only”. Doing so will void the form,
 Once complete you are to return the form to the
person or institution with whom you have entered
into the fiscal transaction concerned.
