AnapplicantwillfillForm49Aonlineandsubmittheform.
Ifthereareanyerrors,rectifythemandresubmittheform.
Aconfirmationscreenwithallthedatafilledbytheapplicantwillbedisplayed.
Theapplicantmayeithereditorconfirmthesame.
Onconfirmation,anacknowledgementwillbedisplayed.Theacknowledgementwillcontainaunique15digitacknowledgementnumber.
Theapplicantisrequestedtosaveandprintthisacknowledgement.
'Individual'applicantsshouldaffixtworecentcolourphotographswithwhitebackground(size3.5cmx2.5cm)inthespaceprovidedintheacknowledgement.
Thephotographsshouldnotbestapledorclippedtotheacknowledgement.TheclarityofimageonPANcardwilldependonthequalityandclarityof
photographaffixedontheacknowledgement.
(h) Signature/LeftThumbImpressionshouldonlybewithintheboxprovidedintheacknowledgement.Thesignatureshouldnotbeonthephotographaffixedon
rightsideoftheform.Incaseofapplicantsotherthan'Individuals',theauthorizedsignatoryshallsigntheacknowledgementandaffixtheappropriatesealor
stamp.Thesignatureshouldnotbeonphotograph.Ifthereisanymarkonphotographsuchthatithinderstheclearvisibilityofthefaceofthe
applicant,theapplicationwillnotbeaccepted.
Signature/Lefthandthumbimpressionshouldbeprovidedacrossthephotoaffixedontheleftsideoftheforminsuchamannerthatportionof
signature/impressionisonphotoaswellasonacknowledgement.
