Practical Exercise 2 : Constructing A Family Genogram


Chosen patient: No.13 (HHBM)
Interviewee: Patient (HHBM) and her daughter (FRBM)
Patient came for regular diabetes (DMT2) follow up. She is
on tablet medication but sometimes forget to take medication
causing her to feel dizzy. Her glucose level was 10.3 mmol/l and
patient has no believe in getting well. She still consumes high-sugar
diet despite doctors warning.

Practical Exercise 3: Preventive Health Care Activities in General


Practice

1. Observed Preventive Health Care Activities


i. Weight Loss Management talk to prevent osteoarthritis
ii. Vaccination for babies, foreigners and students against Hep B and DPT
iii. Advising patient to reduce/stop processed food intake and smoke to
prevent getting cancer (patient has strong family history of multiple cancer)
iv. Advising the patient to restraint touching others as the patient was having
Hand, Foot and Mouth disease
v. Advising about having safe sex to prevent getting STD (patient have active
sexual lifestyle with multiple partners)

2. Description about Weight Loss Management talk to prevent


osteoarthritis
GP advised a patient (69kg, 150cm) to lose weight as she has strong genetic
link of getting osteoarthritis. The doctor further told the patient that by
having an active lifestyle, her life quality will improve and the risk of getting
osteoarthritis and osteoporosis will significantly reduce. The
GP encourages the patient to take part in group (tai chi) exercises or
go to swim with friends. An individualized meal plan was given to the patient
based on several test. The GP assures that she will achieve the ideal weight if
she consistently do exercise and have a balanced diet.
