FILE NOTE (INFORMAL APPEAL) FOR DISCIPLINES


ESSENTIAL INFORMATION

File Notes are the only acceptable means to initiate an informal appeal to a Discipline. Students may lodge a File Note on any issue
which pertains to a unit of study assessment or result. THIS FILE NOTE IS NOT TO BE USED TO APPEAL A SPECIAL
CONSIDERATION/SPECIAL ARRANGEMENT/SHOW CAUSE/PROGRESSION DECISION.

It is the responsibility of each student to be familiar with the policies and processes governing their assessments. Refer to your Unit of
Study Outline in the first instance.

Under Section 3.1.2 (a) of University of Sydney (Student Appeals against Academic Decisions) Rule 2006 informal appeals (File Note)
must be initiated within 15 working days of the date of decision or release of result. File Notes lodged outside of this timeframe may not
be considered, although in extreme circumstances where it can be demonstrated it was impossible to comply with the deadline, the
Sub Dean may use discretion.

File Notes are lodged via email and therefore students do not need to be present in the country to initiate an informal appeal. Being
overseas does not constitute a valid reason for late submission.

Academic judgement can only be made on the basis of the information lodged in this application and any further supporting
documentation provided herewith. Please ensure you have included ALL INFORMATION AND SUPPORTING
DOCUMENTATION/CORRESPONDENCE at the time of submission to facilitate a timely response.

File Notes must be completed on this form. All 3 pages and relevant supporting documentation must be emailed to the relevant Discipline
Administration Office listed below, not to the Unit of Study Coordinator.

File Notes must be saved following the below naming convention:

You must only use your university email account i.e abcd1234@uni.sydney.edu.au to submit your File Note. File Notes received from
any other email will be disregarded. Submitting from your University email account constitutes your electronic signature and validates the
File Note.

Should the File Note pertain to a group assessment, all members of the group must consent to the informal appeal. All group members
must be listed in the File Note. The File Note should only be submitted once; with remaining group members ccd using their University
email accounts.

If the File Note relates to an assessment where hard copy feedback was provided please attach a copy of the assessment and marking to
the submission email.

Students will be notified of the outcome of their File Note via their University email account. Where possible, a decision will be reached
within 10 working days.

Subsequent to the File Note outcome, students have further right of formal appeal to the Director of Academic Appeals. For further
