WHAT ARE THE ADMISSION REQUIREMENTS TO BE SUBMITTED?


A. Required Credentials:
When the applicant passed the PUPCET, he/she must submit an original copy and photocopies of the
following documents to the Office of Admission Services:
For Graduating High School Student

High School Card/Form 138


Certified true copy of Birth Certificate duly authenticated by the National Statistics Office (NSO)
Certification of Good Moral Character issued by the High School Principal/or Guidance Counselor.
Chest X-Ray result with Film.
Medical Clearance issued by the PUP Medical Clinic.
2 in. x 2 in. picture with white background with applicant's name on the photo.

For High School Graduates of past school years


In addition to the above admission requirements:

Certification from high school registrar with school dry seal and noted by the Principal that no copy
of F137A has been sent to other college or university.
Notarized affidavit that applicant did not enroll in any school/college/university within and
outside the country with "waiver" that if there is concealment of previous enrolment,
PUP enrolment becomes null and void.
For PEPT qualifier, a duly authenticated or verified copy of the Certificate of Rating and a
Certification issued by the Department of Education (CMO No.15,s. 2003)
For an ALS A&E Test qualifier, a certification of test result and a certification of completion for
secondary level.

B. Other Requirements:
1. Two (2) pcs. 2 x 2 colored photo with white background and nameplate.
2. One (1) piece long, brown envelope.
3. For applicants whose 4th year High School English final grade is below 80%, PEPT and ALS A & E Test
qualifier, proceed to the Department of English, College of Arts and Letters, Room S-411, 4th Floor,
South Wing, PUP Sta. Mesa Campus to take the English Placement Test (EPT). A fee of P150.00 for
the English Placement Test shall be paid to the Cashier.
4. For entrance scholars, Entrance Scholarship Committee Route and Approval Sheet duly signed by the
members of the concerned committee and one (1) photocopy of the credentials submitted for
scholarship grant.

Step 1. SUBMISSION OF ALL REQUIRED CREDENTIALS FOR ADMISSION


Submit all the REQUIRED CREDENTIALS FOR ADMISSION to the Admission Officer, Office of
Admission Services (OAS/OUR), Ground Floor, W-107 West Wing, Sta. Mesa, Manila, on the date of your
scheduled processing of credentials, interview and enrolment as reflected in your SAR Form 1.

Only applicants with complete credentials required for admission will be accommodated and shall go
through the admission process.

Freshman applicants are advised to come for interview in appropriate attire and proper grooming (with
collar and sleeves; no rubber slippers, tokong/shorts; and no earrings for male).

Non-appearance on your scheduled date of interview would mean automatic forfeiture of your
slot in the course of your choice, or even forfeiture of the slot in the University depending on the turnout
of PUPCET qualifier. Admission of qualified applicants is on a FIRST-COME-FIRST-SERVED basis.
However, late applications may be rescheduled or put on the waiting list subject to the availability of
slots.

For applicants whose 4th year High School English final grade is below 80%, PEPT and ALS A &
E Test qualifier, proceed to the Department of English, College of Arts and Letters, Room S-411, 4th
Floor, South Wing, PUP Sta. Mesa Campus to take the English Placement Test (EPT). A fee of
P150.00 for the English Placement Test shall be paid to the Cashier.

Step 2. COLLEGE INTERVIEW AND REGISTRATION


Proceed to the College offering the program of your first choice for interview by the Dean or
Chairperson. In case you are not qualified or there is no more available slot in your first choice, you can go to
the college of your second or third choice until you get the approval for acceptance in your desired program.

Admission in PUP is on a FIRST-COME-FIRST-SERVED basis subject to the availability of slots.
In addition to the general criteria set by the University, admission program depends on the satisfaction
of the specific criteria set by the Colleges for Academic Year 2015-2016.

Step 3. MEDICAL EXAMINATION


Proceed to the PUP Medical Clinic, Ground Floor, East Wing, and present your Student Admission
Record Form 1 (SAR F1) to secure a medical clearance.
Step 4. PAYMENT OF TUITION AND MISCELLANEOUS FEES
Pay the full or 50% of your total assessed fees (tuition and miscellaneous fees) indicated in the Route
and Approval Slip at the PUP Cashiers Office, Ground Floor South Wing.
NOTE: To avoid cancellation of slot, you are advised to pay your tuition/miscellaneous fees within the
enrolment period.
Step 5. ISSUANCE OF REGISTRATION CERTIFICATE
After payment, submit your accomplished SAR Form 1 to PUP Admission Services/Office of the
University Registrar (AS/OUR), Ground Floor, West Wing to get the printed copy of your Registration
Certificate from May 11 to May 21, 2015. Proceed to your assigned Admission Staff base on the first letter of
your surname.
NOTE: Present your Registration Certificate to your teachers during the first day of classes.
Step 6. ISSUANCE OF STUDENT ID
Proceed to the Office of the Director, Student Services at Rm 208 Charlie Del Rosario Hall for the
processing of your Student ID application after the issuance of your registration certificate from May 11 to May
21, 2015.
