1. To claim that Philhealth Check payable to me representing
the proceeds of my Philhealth benefit for the hospitalization

admitted at the Philippine General Hospital from January 5
to January 14, 2006;
2. To transact, file, follow-up and sign for and in my behalf all
papers and documents necessary for the said claim;
3. To encash said Philhealth Check for and in my behalf;
4. To receive and acknowledge receipts and generally to do or
perform any act or deed that may be deemed necessary in
furtherance of this authority.
HEREBY GIVING AND GRANTING unto said Attorney-in-Fact full authority to do
or perform every act and thing requisite or necessary to be done in and about the
premises as fully to all intents and purposes as I might or could do if personally present
and ratifying and confirming all that my said Attorney- in-Fact shall lawfully do or cause
