16. Instructions
1. This application should be completed in block letters, legibly and neatly.
Applying Level 1, 2 and 3
2. To apply for a subject at a higher stage, all subjects at the previous stages should be passed, exempted or applied for
Simultaneously up to Maximum of 06 subjects.
3. Those who are eligible to apply for exemptions have to submit the duly completed Exemption Application Form before submitting the
Examination Application Form.
4. Only ONE medium in Foundation (Level I) and Operational Level (Level 2) can be applied for. Care should be taken to fill the
appropriate column having the subject/s.
There will be NO change to the medium applied for.
Applying for Core Subjects
5. MAF (1) should have been passed / exempted before applying for OMA (7).
6. OMA (7) should have been passed / exempted before applying for IMA (13).
7. FAF (2) should have been passed / exempted before applying for FAR (8).
8. FAR (8) should have been passed / exempted before applying for CRA (13).
9. BE I (6) should have been passed / exempted before applying for BE II (11).
10. BE II (11) should have been passed / exempted before applying for PC (16).
11. Students are allowed to take BOTH BLT & CLT together
Applying for Strategic Level
12. To apply for Strategic Level, all subjects at the previous stages should have been passed or exempted.
13. Any number of subjects in the Strategic Level can be applied even in the first sitting. Referred candidates need to take only the referred
subject.
14. Those who have completed upto Professional I (P I) and not sat for PII in the Previous syllabus are required to sit Integrative Management
Accounting (IMA) at Managerial Level. Strategic Level Subjects (except Business Strategy & Policy) are allowed to apply ONLY after
completion of IMA.
15. Candidates applying for the Integrative Case Study (ICS) can do so only after completing the Four subjects at the Strategic Level
16. A maximum of 6 subjects could be applied for at one sitting. (Applicable upto Managerial Level Only)
17. This application form should be used to apply for the November 2015 Examination only.

17. Important Notes

Examination fees need to be pay before 1st September 2015 and penalty rate of Rs.500/- will be charged for payment between
1st September 2015 to 15th September 2015
The Special Payment Voucher used for payment of Examination Fee should not be used to make any other payment.
Only Candidates who have paid their Annual Subscription Fees for year 2015 are eligible to apply for the Examination.
It is essential that you provide contact details i.e. Telephone Number(s) or e-mail or fax number
Ensure that your application is received on time at the CMA secretariat.
Payment will not be refunded/transered under any circumstances
Applications, which do not conform to instructions given and those
incorrectly completed,
not fully completed,
completed in illegible / clumsy hand writing,
not properly attested,
submitted without a duly endorsed SPV for the correct amount of Examination Fee
will be REJECTED.
