The purpose of completing this JOB DESCRIPTION QUESTIONNAIRE is to enable the


Human Resources to prepare or update a job description and to evaluate if the job is
as per organizational structure and requirement. You are the best information source
about your own job. It may have existed for many years, but it certainly has changed
to some degree from the last time it was evaluated.
For filling this form consider how your work relates to other employees
responsibilities in your department, to Departments and to the Vendors / Clients. You
and your supervisor may wish to work together to identify the most important
responsibilities of your job.
As you complete the Questionnaire, reflect on the following:

KRA and Attribute.
The overall finalized score for each individual in the function will then ranked and compared
for normalization. The normalization process for finalizing the rating will be based on the
achievement score and behavioral indicators as exhibited by the individual as defined in the
rating definition sheet.
