The Commissioner for Cooperation & Registrar of Cooperative Societies, A.P.
Hyderabad, in his letter third read above, has forwarded the medical claim in respect of Sri
Sandeep Kumar Sultania, I.A.S., Commissioner for Cooperation & Registrar of Cooperative
Societies, A.P. Hyderabad, for Rs.7691.78 (Rupees Seven Thousand Six Hundred and Ninety
One and Seventy Eight Paise only) towards reimbursement of medical expenses incurred by
him for the treatment of his daughter Vedika Sultania, in Vaidyavidhana Parishad Civil
Dispensary, Secretariat, Hyderabad, from 28.07.2014 to 20.10.2014 for necessary sanction
orders. The MOS has furnished application in the format prescribed, together with Emergency
Certificate, Declaration Certificate, Non Drawal Certificate, and Essentiality Certificate.

The Civil Surgeon Specialist, Govt. Civil Dispensary, Secretariat, Hyderabad, who has
scrutinized the bills has certified the admissible amount as Rs.6292/- ( Rupees Six Thousand,
two Hundred and Ninety Two only) vide letter fourth read above.

Therefore, the Government, after careful examination, hereby accord sanction for the
payment of the admissible amount of Rs.6292/- ( Rupees Six Thousand, two Hundred and
Ninety Two only) to Sri Sandeep Kumar Sultania, I.A.S., Commissioner for Cooperation &
Registrar of Cooperative Societies, A.P. Hyderabad, towards reimbursement of the medical
expenses incurred by him for the treatment of his daughter Vedika Sultania as certified by the
Civil Surgeon Specialist, Civil Dispensary, A.P.Secretariat, Hyderabad in terms of the orders in
the references first and second read above.

The Spl.Commissioner for Cooperation & Registrar of Cooperative Societies, A.P.
Hyderabad, shall draw and disburse the amount sanctioned in para (3) above to Sri Sandeep
Kumar Sultania, I.A.S., duly debiting the expenditure to the Head of Account from which the pay
and allowances of the Member of Service are drawn.

This order does not require the concurrence of Finance Department as per the orders in
force on the subject.
