KIDS PASSION AND FASHION CONTEST REGISTRATION & CONSENT FORM

Dear Parent/Guardian:
Kabayan Weekly and Ansar Mall will organize Kabayan Kids Passion and Fashion Contest at 4 p.m. on Saturday 29 of
August 2015 that will feature childrens passion and fashion. We are seeking ages 4 to 12 to participate in fashion contest
and teens below 16 years old to participate in Kids & Teens Voice or KTV.
In this contest event, there is no cost to participate. Please read and fill out the form in order for your child to be a
contestant on this event. If you have any questions regarding your child as a participant, you may contact Kabayan
Weekly Office.

PARENTAL/GUARDIAN CONSENT FORM

1. I agree to participate my child in the KABAYAN KIDS PASSION AND FASHION CONTEST presented by Ansar Mall and
Kabayan Weekly.
2. I give the permission for my child to be photographed, videotaped, and/or interviewed in connection with this event.
3. I give the permission to the show KABAYAN KIDS PASSION AND FASHION CONTEST for public relations purposes,
including print and online communications media.
4. I authorize the use and production of any/all photographs and video record taken of my child, without compensation to
me/my child. All the photographs/video recordings shall be the property of the KABAYAN KIDS PASSION AND FASHION
CONTEST organizer. I waive any right to inspect or approve the finished photographs/videotapes, or printed material that
may be used in conjunction with them.
5. I agree that my childs image and short bio to be published in the Kabayan Weekly newspaper and online post.
I voluntarily agree to the terms set forth herein and acknowledge and agree that my child and myself are bound by the
Official Contest Rules and I am bound by the terms of this parental consent
