Clinic is a health care facility that organizes personal health services , provide basic medical care and

held by more than one type of health personnel (nurses or midwives) and led by a medical personnel
(doctors, specialists, dentists or dental specialist).

Based on the type of service, the clinic is divided into Primary and Secondary Clinic
Secondary. Both kinds of these clinics can be organized by the government, local government
or community.
Primary clinics are clinics that administer basic medical services.
Secondary Clinic is a clinic which organizes specialized medical care or basic and specialistic medical
services. The nature of health services can be organized outpatient, one day care, hospitalization
and / or home care.
Clinic building at least consist of:
a. registration room / waiting lounge area;
b. doctor's consulting room;
c. administrative space;
d. space action;
e. pharmaceutical space;
f. bathroom / wc;
Infrastructure include:
a. installation of water;
b. electrical installation;
c. installation of air circulation;
d. waste management facilities;
e. fire prevention and suppression;
f. ambulances,; and
g. other facilities as needed.
In addition, the clinic should be equipped with adequate medical and nonmedical according to the
type of services provided.
Terms such equipment are:
1. meet quality standards, security, and safety.
2. have a marketing authorization.
3. Must be tested and calibrated periodically by the Central Safety and Health Facilities / or institutions

LEADER
Primary CLINICS
(1) Primary Leadership Clinic is a physician or dentist.

(2) Medical personnel at the Primary Clinic consists of a minimum of two (2) people
doctor and / or dentist.
SECONDARY CLINICS
(1) Leadership in Secondary Clinic is a specialist or dentist specialist competence in accordance with
the type of clinic.
(2) Medical personnel at the Secondary Clinic consists of a minimum of 1 (one) specialist from each
specialization according to the type of service that is offered.
(3) Secondary Clinic can employ doctors and / or dentists as executive personnel of medical
services.
PRACTICE LICENSE
(1) Every medical personnel practicing in the clinic must have Certificate of Registration and License
to Practice (SIP) in accordance with laws and regulations.
(2) Every other health professionals who work in the clinic must have a Permit as registration marks /
Registration Certificate and Work Permit (SIK) or Permit Practice Pharmacists (SIPA) in accordance
with the legislation.
LICENSING
(1) To establish and organize clinics must obtain permission from the local government district / city
after getting a recommendation from health districts / municipality.
(2) The Department of Health district / city issued a recommendation referred to in paragraph (1) after
the clinic meets the provisions of clinical requirements in this Regulation.
(3) The license application submitted by attaching the clinic:
a. a letter of recommendation from the local health department;
b. copy / photocopy establishment of business entities except for individual ownership;
c. Full identity of the applicant;
d. a certificate of approval from the location of the local government;
e. proof of ownership or land use or permit the use of a building for the implementation of activities for
belonging to a private or a contract for a minimum of five (5) years for renting the building for the
implementation of activities;
f. document Environmental Management Effort (UKL) and Environmental Monitoring Effort (UPL);
g. clinical profile to be established includes organizational structure management, health personnel,
infrastructure, and
equipment and services provided; and
h. Other administrative requirements in accordance with the provisions of the legislation.
i. Clinic permission granted for a period of 5 (five) years and can be extended to apply for an
extension of six (6) months prior to the expiration of the license.
HOSPITAL SERVICES
(1) Clinic which organizes inpatient services must provide:
a. inpatient unit that meets the requirements;
b. patient beds at least 5 (five) and a maximum ten (10);
c. medical and nursing staff according to the number and qualifications;
d. nutritional personel, analyst, pharmacy personnel and health workers and / or other appropriate

non-health personnel needs;


e. kitchen for nutrition unit;
f. Primary Clinical laboratory services.
(2) Inpatient treatment can only be done for a maximum of 5 (five) days.
Based on the Regulation of the Minister of Health (Permenkes) No. 28 In 2011, one of the
requirements of permits and organizing clinics must attach documents Environmental Management
Effort (UKL) and Environmental Monitoring Effort (UPL); Because the clinic is a business and / or
specific activities that have an impact on the environment. Each clinic produces medical and nonmedical waste in the form of solid or liquid. The Department of Health held a meeting between the
owners of Private Health Facilities to do Perajnjian Cooperation in the framework of the Medical
Waste Disposal. ,
