Let's talk about dying - Peter Saul
Summary
The TED Talk by Peter Saul is based on the research that was conducted in
Newcastle Australia working with people who were dying and their
families.

We cant control if well die, but we can occupy death, in the words of Dr.
Peter Saul.
He shows that how we die is changing and the modern ways arent
good. Most intensivists agree, the last thing they want is to die in ICU.
He opened up with a very moving statement that when 2000 brain cells
die daily he argues that our dying process really truly has started. Working
for 30 years he says he has seen many changes, the most significant one
was in the role of technology. Technology, he argues, is credited with life
prolonging, rather than life saving. This increased prolonged existence
leads to longer life; not necessarily better life. He also states that chronic
diseases are now taking control in the dying process.
He claimed that the four ways to die are: (a) sudden death, (b) terminal
illness, (c) organ failure, and (d) frailty. Sudden death is declining due to
technology and increased education. Terminal illness is usually reserved
for the younger person. Organ failure will lead people into the Intensive
Care Unit of the local hospital. The final way to die, frailty, is what most
people will experiences. Life longevity raises frailty. This diminishing
capacity is an inevitable part of aging.
He summarized his talk by imposing the two most important questions
regarding the dying 1. What plan if any do you have for your impending death?
2. In the event that you would become too sick to speak for yourself have
you chosen someone to speak for you?

Perspective
Death may be one of the biggest unknowns, but we all do know what
happens during the dying process leading up to natural death.
Understanding this process, even at the cellular level, and familiarizing
ourselves with images of dying may make us more comfortable with the
idea of death.
I completely agree with Dr. Saul and believe that instead of avoiding this
subject, we should speak to our loved ones on this and make it a loving
subject.
With rising longevity and the consequent rise in natural death, will 100
become the new 50?
This is a quote from Dame Cicely Saunders, who he met when he was a
medical student. She founded the hospice movement. And she said, You
matter because you are, and you matter to the last moment of your life.
And I firmly believe that thats the message that we have to carry forward.
We matter till the last moment of our lives, lets not ignore the last phase
of our lives so heres the pitch, I think it's important that we don't just get
on this freeway to ICU without thinking hard about whether or not that's
where we all want to end up, particularly as we become older and
increasingly weak, ICU has less and less and less to offer us. There has to
be a little side road off there for people who don't want to go on that track
including me. Theres a small thing we can do about what could happen.
The small idea is, let's all of us engage more with this in a way that Mr.
Saul has explained. Why can't we have these kinds of conversations with
our own elders and people who might be approaching this? We should ask

patients the two questions, imposed by Dr. Saul in the end of his talk
which can really produce amazing outcomes and make it easier for the
people wo are going to surviving after someones death and also for the
patient that is approaching this.
