Formative Assessment #1
Grade: 2nd Grade Social Studies
Standard:
8.4.2.C: Identify how cultures have commemorations and
remembrances.
Choice Boards:
On the board students can choose how they want to prove that they
have mastered the world cultures standard.
Objective:
Using a project of their choice, students will showcase their
understanding a world cultures commemorations and remembrance.
Students can choose from
A paper pencil quiz
A brochure
A diagram
A play/skit
A paper
Evaluation:
A specific rubric will be provided for each off these individual
assignments.
Modification:
Student with a writing problem can talk into the computer and the
computer can write what the student is saying into essay form.
How is this Differentiated?
Students get to pick multiple different aspects to be assessed on.
Student can choose the way they are assessed; the student also gets
to pick what culture they are being assessed on. This is formative
because it is an on-going assessment, shows what the student knows
about the specific culture.

Grade: 4 Science
Standard:
Objective:
Using a quiz of their choice, students will demonstrate their
understanding of the.
Choices: Students can choose the format of a 20-point quiz that they
have to take.
#1 quiz: One Question multiple step oral quiz
#2 quiz: Three open-ended questions quiz
#3 quiz: One question essay quiz
#4 quiz: Fifteen question traditional quiz
Evaluation:
There will be a rubric created and presented to the students before and
during quizzes 1-3. Points will be labeled throughout Quiz number 4.
Accommodation:
A student that has a writing disability can take the oral quiz. The
student can also type quiz 2,3, and 4 if needed.
How is it Differentiated?
Students get to choose to vehicle of the 20-point quiz.

Grade 1
Standard:
Objective:
Using an assessment of their choice, students will showcase their
understanding of careers in the community.
Choices:
Role Play: Students will choose a career and they will have to play the
role of the career that they chose.
Jigsaw: Create expert groups where everyone is assigned a career, the
students will be able to explain the role the person they have chose
within their small groups of students. The small group will then create
a diagram of the careers discussed in their group.
Picture Panels: Students will draw the career, then the students will
caption their picture with the job description.
Evaluation:
A rubric will be created for all three assessments that specify what
each assessment needs to cover.
Accommodation:
A scribe will be provided for students that cannot write on their own.
How is it Differentiated?
Students can choose the assessment that they believe will better
showcase their understanding of careers in the community.

Formative Assessment #4
Grade K: Math
Standard:
Amounts to 10.
Objective:
Using concrete, pictorial, and abstract objects Students will be able to
match numerals to given sets 0-10.
Benchmark:
All students will start at #1 students that score at least a 80% will
move on to #2, students that score at least an 80% on number 2 will
move on to #3.
#1 Using counters student will count out manipulative that the
numeral has on the card.
#2 Draw the set to the numeral 0-10
#3 Write the numeral to match the picture set.
Evaluation:
Teacher will check off the numbers that the student got correct.
Accommodation:
Instead of writing the numeral, student will verbally tell the teacher
what the number is.
How is it Differentiated?
Students are assessed on the same standard, those who accomplish
the first step move on to harder steps and greater numbers and value
of sets.
