TRAUMATISMO ESPLENICO

Recin en la dcada del 50, se relacion al bazo con una funcin especfica, cual
es de tipo inmunolgico (King y Shumacker). (1) Por su especial circulacin, de
tipo abierta, la sangre entra en ntimo contacto con el tejido retculo - endotelial
(pulpa roja) y con clulas inmunocompetentes (pulpa blanca). Por esta va, este
rgano acta como un filtro biolgico removiendo de la circulacin ciertos
antgenos, bacterias y clulas alteradas senescentes. Adems, el bazo es
productor de inmunoglobulinas (igm), de una protena opsnica, la tufsina, que
aumenta la capacidad bactericida y fagoctica de los neutrfilos y de properdina,
un componente bsico de la va alternativa de la activacin del complemento. (2,

As, se explica el riesgo de sepsis fulminante en individuos esplenectomizados,
cuya incidencia en casos traumticos alcanza hasta el 1 %, lo que significa un
riesgo entre 58 y 200 veces mayor que para la poblacin general. Esta sepsis
puede ocurrir meses aos despus de una esplenectoma e involucra
microorganismos como Streptococcus pneumoniae (50 %), Haemophilus
influenzae, Neisseria meningitidis y E. Coli. (4, 7)
Esto ha llevado a tratar de preservar el bazo en cuanto sea posible. Ello se
traduce en que actualmente frente a los traumatismos esplnicos se pueda optar
en forma segura entre al menos
tres conductas: no operatoria, ciruga
conservadora y esplenectoma.
DIAGNSTICO.Siendo el bazo uno los rganos ms frecuentemente lesionados en un
traumatismo abdominal (25% a 40% en contusiones) y (2% a 4% en trauma
abierto), (3, 8, 9), debe siempre sospecharse su compromiso. Ello es
especialmente vlido ante la evidencia de impactos por el costado izquierdo,
sealndose que hasta un 20% de los pacientes con fracturas costales izquierdas
bajas (ltimas seis) presentan lesin de este rgano. (3)
Indudablemente, su manifestacin es a travs de un hemoperitoneo la mayor parte
de las veces (excepto en casos de hematomas subcapsulares intra
parenquimatosos localizados) y ser la magnitud de ste lo que determine el
espectro clnico de presentacin: desde un paciente asintomtico mnimos
