I, MARIE ANNE FERRANDO DELA CRUZ-PIDO, of legal age, Filipino and
resident of Brgy. Central, Tarragona, Davao Oriental, Philippines, after having been duly
sworn to in accordance with law, do hereby depose and say:
That I was born on February 22, 1965 at Holy Cross, Manay, Davao Oriental to
spouses Tirso Dela Cruz and Purificacion Ferrando both deceased;
That it appears on my Certificate of Live Birth duly issued in the National
Statistic Office (NSO) my first name appears MARIE ANNE and also my date of birth it
appears February 22, 1965 while on Professional Regulation Commission my first name
was entered MARY ANN and also my date of birth was entered February 4, 1965;
That since in my grade school I am using my First Name as MARY ANN and
Date of Birth as February 4, 1965 until I became a teacher because my mother told me
that my first name is Mary Ann and my date of birth is February 4, 1965;
That I did not correct my name after taking the examination because I did not
notice and I recently knew and get my authenticated birth certificate in the National
Statistics Office (NSO) and due to negligence the correction of my first name and date of
birth was lapse.
That I am executing this affidavit to attest to the truth of the foregoing statements
and to inform the Philippine Regulatory Commission and for whatever legal purpose this
may serve.
