and by virtue of the laws of the Republic of the Philippines, with office address at

as position in the company, after duly sworn to in accordance with the law depose and
say:
1. THAT, NAME OF IMPORTER ENTERPRISE, will be the direct importer of
volume in words (_______) metric tons of specify flat steel input to be imported,
with complete specifications, from Japan, for consumption for the year ____;
2. THAT, the volume in words (_______) metric tons of specify flat steel input to be
imported shall not be for resale or trading; and that it shall be exclusively used as
direct raw material by NAME OF IMPORTER ENTERPRISE for the manufacture
of volume in words (_______) metric tons of specify the importers products
declared in its registration application, and;
3. THAT, the total quantity of volume in words (_______) metric tons of specify flat
steel input to be imported, applied to be imported under the JPEPA-TRQ Scheme
cannot be sourced locally in the required quantity, quality/ specifications, at
reasonable and competitive price and within the delivery date required.
4. THAT, the company has not yet opened the LC or made any other form of
payment for the above importation.

1. Photocopy of the DTI letter notifying the approval of the importers
application for accreditation under the Scheme;
2. Importers Notarized Affidavit of Intent as final user indicating endusage and application, among other things (DTI Form TRQ-03);
3. Photocopy of sales contract, sales/purchase order, pro-forma invoice,
or like documents, covering the importation, issued in the name of the
importer as consignee to whom the shipment will be released by the
BOC; provided further that the applicable Japanese Industrial
Standards (JIS ) to which the
TP, HR and CR steel being imported
conform to (as specified by the importer) is indicted in the said sales
contract, sales/purchase order, pro-forma invoice, or like documents,
covering the importation;
4. A written explanation by the importer substantiating his case in the
event of a controversy.
