CHECKLIST: Visit of family/friend/boy-/girlfriend

Documents must be submitted in the same order as in the checklist. Check the boxes below, sign
and submit the checklist with the application.

Cover letter from the Application portal or application form (if you are not applying online)
signed by the applicant.
Passport photo. (not older than 3 months, 4 x 3 cm in size, white background, with no
glasses and glued to the cover letter.)
Original passport with validity period of at least 3 months after the projected expiry of
the visa. (Both new and old passport(s) if any)
Photocopy of passport (all pages that contain personal information, stamps, previous visas
and other permits).
Original travel insurance + one copy (must be valid for the stay + a period of minimum 15
days after expected return and covering the entire Schengen area for a minimum amount of
EUR 30 000 for the duration applied for (as per travel plan submitted).
Booking of a roundtrip air ticket (do not buy the ticket until a visa has been granted)
Completed Itinerary to Schengen countries form (available on the Embassys website),
with fixed travel dates of maximum 90 days.
Bank statement for at least the last 3 months (original bank book + 1 copy of each used
page. If insufficient, the reference must provide financial guarantees, see information
below.
Proof/justification of relation and documentation of civil status (relevant birth certificates
for family visits, marriage certificate, family registration document)
Questionnaire for visiting fianc/boyfriend/girlfriend or questionnaire for visiting friend.

Original documents confirming employment, private business, school or studies;
permission for leave of absence.
For minors only (>18yrs) Birth certificate and parental authorization from both parents.

Documentation from the reference/sponsor

Original and signed invitation letter from reference in Norway stating the purpose of the
visit and the duration and period of your intended stay. For friend/boy-/girlfriend visit the
letter should also state the type and length of the relationship.
Original Guarantee Form for Visit attested by the Norwegian police + 1 copy (Only
required if the applicant does not have documents proving sustainable economic means or
income.)
OR, if the reference person/sponsor resides- and have a valid residence permit, in
Indonesia documents proving sustainable economic means or income is normally
sufficient.
Copy of relevant pages of the reference person`s passport ( Norwegian residence permit or
previous visas to Indonesia or current residence permit in Indonesia)
Documentation of civil status

NOTE:
Documentation from reference person must be send by regular or express mail directly to
the applicant before applying, and not by fax or as a scanned attachment to e-mail. Please
do not send any documents directly to the Embassy, as these would not be taken into
consideration.
Letter granting Power of attorney (Has to be submitted for applicants who want the
reference or other person to represent them in the case. Separated form is obtainable on the
Embassys website.)

IMPORTANT
I hereby confirm that I personally have submitted this application for a visa and that all
information stated in the application and the supporting documentation is true. I am aware that
providing incorrect information or falsified documents deliberately are punishable acts by
Norwegian law and will lead to refusal of the visa application.
Im aware that it is my own responsibility to enclose all required documents according to the
checklist when I submit the application. Documents submitted after the application date will not
be taken into consideration by the Embassy.
