Differentiation of the Twenty-eight Pulses

The Category of the Slow Pulse (Ch Mi, r)

Consists of slow, moderate, rough, and bound pulses, which are all slow.
The characteristic of the slow pulses: The frequency of the pulse beat is slow,
causing a reduction in the pulse rate.
As for the depth, the pulse can be felt at all three levels (superficial, middle,
and deep).

The slow pulse arrives and

departs slowly. It beats
three times per respiration.

The moderate pulse is

slightly faster than the slow
pulse. It beats four times
per respiration, and its arrival is leisurely.

The rough pulse is slow,

thin, and short. It has a
rough onset and recession.

The Guiding Principles of Pulse Manifestation

The Category of the Rapid Pulse (Shu Mi, x)

Consists of rapid, racing, skipping, and stirred pulses, which are all fast.
The characteristic of the rapid pulses: The frequency of the pulse is fast,
therefore causing the rate of the pulse to be excessively high.
This type of pulse can be palpated at all three levels (superficial, middle, and
deep).

The rapid pulse beats five

(or more) times per
respiration.

The racing pulse beats

seven or eight times per
respiration; the arrival of the
pulse is hurried.

The skipping pulse is rapid

and pauses periodically with
a fixed rhythm (skips at
irregular intervals).

The stirred pulse is slippery,

rapid, and forceful. Shaped
like a bean, it spins and vacillates.

a result of dry feces adhering to and stagnating in the intestinal tract. The main
cause of disease is abiding food.
If there is retention and stagnation of abiding food in the upper portion of the
digestive tract, there will be vomiting; this condition should be treated with
emesis.

Chapter 13: Discussion of the Pulse and Signs

of Fright Palpitations, Nosebleed, Blood Ejection,
Precipitation of Blood, Chest Fullness,
and Static Blood
Key points emphasized
-- Discusses the pulse, signs, and treatment of fright palpitations, nosebleeds,
blood ejection, precipitation of blood, chest fullness, and static blood.
-- If the wrist pulse manifestation is stirred and weak, the stirred quality indicates fright, while the weakness indicates palpitations.
-- If the instep yang pulse manifests as faint and floating, the floating and forceless quality signifies stomach qi vacuity, whereas the faint quality indicates
inability to eat. This is the pulse manifestation of fear and dread pattern,
which is caused by fright and fear, and characterized by unclear vision and a
pulse that pauses temporarily and then resumes.
Question: If the wrist pulse is tight and the instep yang pulse is floating and forceless, this
indicates stomach qi vacuity. A sunken and tight wrist pulse reveals cold repletion; cold
evil congests and gathers in the upper burner and inevitably causes distention and fullness
of the chest and belching. In patients with stomach qi vacuity, if the instep yang pulse
manifests as floating and forceless and the lesser yang pulse (sho yng mi, }) is
tight and stringlike, there will be heart palpitations. What is the cause?
Answer: This is a result of cold evil and water--damp contending with each other and
binding together to cause obstruction in the heart vessels, giving rise to heart palpitations.
If pulse diagnosis reveals a rough, soggy, and weak pulse manifestation, this indicates
great loss of blood.

A stringlike and large pulse is the pulse manifestation of vacuity taxation due to
essence and blood depletion. Although the pulse is stringlike, its strength is reduced under heavy pressure. The pulse is large, yet empty. A pulse that becomes

weak under heavy pressure indicates cold; a large, empty pulse indicates vacuity. This pulse signifies a combination of vacuity and cold, and is known as a
drumskin pulse. The presence of a drumskin pulse in women reveals a tendency
for late miscarriage or incessant spotting of menstrual blood. In men, a drumskin pulse reveals blood loss.
Patients with blood loss should not be treated with diaphoresis (to resolve
the exterior). If the sweating causes severe damage to both yang qi and yin humor, there will be a fear of cold and shivering of the whole body.

Question: If there were bleeding from the nose that continued for many days, what would
the pulse manifestation be like?
Answer: When palpating the pulse, it would feel soft and forceless in the superficial and
middle levels, and the cubit (ch1) position would be floating, large, and forceless. In addition, the eyeballs would be dark yellow in color. These are indications that the bleeding
