the BREEDER hereby TRANSFERS AND CONVEYS all his rights over the
aforesaid dog to the ADOPTER, subject to the terms, conditions, and
warranties stated hereunder.
3. That the BREEDER warrants that the dog has no known illnesses or health
deficiencies, that the dog is healthy at the time of sale, and that
appropriate shots for the dogs age have been given and a record of this
vaccination history is included and on file at :

4. That the dog is sold with the understanding that the ADOPTER agrees
that the dog would become part of his/her family and reside at the home
of the ADOPTER.
5. That the ADOPTER agrees to take proper care of the dog, and for this
purpose, shall give the dog adequate nourishment and ensure that the dog
is timely given the necessary vaccination shots.

6. That the ADOPTER agrees that if any time, and for any reason, he/she is
unable to keep or properly care for the dog, the ADOPTER will contact the
BREEDER and the BREEDER will have the first option to either: (1) take
the dog back, or (2) assist the ADOPTER in finding a suitable home for the
dog. Should the ADOPTER violate this provision, he shall be liable to pay
damages

BREEDER

responsible for any damage, accident, or injury that may result from the

8. That should the dog become ill or unsound, any and all cost of treatment
will be the responsibility of the ADOPTER and not be reimbursed by the
BREEDER.
9. That no other warranties or guarantees, expressed or implied, are made
under this contract except as stated above.
10. That any suit arising from this contract shall be filed exclusively in the
courts of competent jurisdiction in the Province/ City / Municipality of
