Completed Form 157A (the most recent) (Signed by applicant and all persons over the
age of 18 included in the application. If applicant under 18 years of age signed by
parent/guardian) The form is available on www.immi.gov.au
Valid passport (ie valid on the date of intended initial entry to Australia)
Six (6) recent passport photo for all persons included in the application
Proof of main applicants 576 visa
Letter from the AUSAID counselor from institution, where the main applicant is
studying regarding family entry
Proof of medicals conducted by Panel Doctor (copy of receipt to be provided,
medicals will be forwarded by the panel doctor). All medicals not undertaken in
South Africa or Namibia will be referred to the Health Assessment Services for
comment
HIV test required for student visa applicants from Sub-saharan Africa aged 15 and
above seeking a total stay of greater than 12 months
Certified copies of Birth Certificates or original affidavit of birth stating applicants full
names, date and place of birth and names of both parents. Children under the age of
18 a full birth certificate stating both parents names is required
Certified copy of marriage certificate or evidence of defacto relationship
Child/ren travelling with one parent only - certified permission from other parent that
child/ren can be granted a visa for the requested period or evidence that other parent has
no custodial or access rights (eg death certificate or court order)
Funds is not fully covered by AUSAID stipend - please provide proof of additional
funds to cover living & educational expenses (even if they are not intending to travel
to Australia with you) see second page for details
Overseas student health cover for at least 12 months or length of course is less than 12
months for family members
Proof of enrolment of school-age dependants

If family accompanying you to Australia (more than one child)

A money deposit or the value of an item of property held by the applicant or by a

person who is providing support to the applicant;
A loan from a financial institution;
A loan from the government of the applicants home country;
Financial support (such as a scholarship) from:
(i)
the government of a foreign country
(ii)
the Commonwealth Government, or the government of a State or Territory; or
(iii)
a business; or
(iv)
a multilateral agency
If another person is providing funds to support your application. Please provide a letter
of support from your sponsor
A declaration by the applicant stating that he or she has access to funds from an
acceptable source that is sufficient to meet course fees, living costs and school costs for
the remainder of the scholarship.
Bank statements and / or salary notification from your employer

Verification of bank statements

We require the original or certified copies of latest bank account/s statements for yourself or your
sponsor prior to your application. Please provide permission from the account holder for the
verification of the bank statements. The name of the Chief Inspector of the bank and direct
contact details are also required. If we are unable to verify the funds we are unable to consider
them as proof of financial capacity.
