Introduction:

Nursing practice has changed greatly7 in recent years with regards to the kind of career
opportunities available to professional nurses. Not only is there a greater variety of nursing
services, but there are also many different ways in which a nurse can practice within a given area
of nursing.
There was time when professional nurses had very little choice of services because most
nursing was centered in the hospital and bed side nursing. But now working situation for nurses
is very different today.
The demand for qualified and skilled nurses never diminishes but continues to grow with
the expansion of health care facilities. Apart from jobs in hospitals, clinics, nursing homes,
orphanages, old age homes, there is also scope for private practice. With the expansion of
educational and training institutions in nursing, the demand for teachers is also increasing.
Nurses spend much of their time on their feet, either walking or standing. Nursing is also an
emotionally demanding job, involving long and strenuous working hours.

Meaning of the key terms:

Career: the particular occupation for which you are trained

Career opportunity: a favorable or advantageous circumstances or combination of


circumstances. This is very general meaning and can mean different to different
peoples.
Profession: an occupation requiring special education
Professional advancement: gradual improvement or growth or development in

1. Hospitals Nursing Services:
The function of professional nurse in hospital is usually work broader than any other kind
of nursing. Once the place of acutely ill only, the hospital are generally become a health
center which offers comprehensive health services to the patient, his family and the
community complete community health ad family planning services are often include in what
is offered by the general hospital. Thus, the nurse in the hospital actively involved in many
different ways of meeting all needs of the patient, physically, emotionally, spiritually and
socially. The nurse may participate in direct nursing care and bed side nursing, health
planning, healthy teaching, planning for care in the home, rehabilitation services to the out
patient or he may assign these duties to others and be responsible for administrative duties
and supervising those who practice in the clinical area. If the hospital is a teaching institution
the staff nurse may also be involved in clinical supervision of the nursing students.
Functions performed by the professional nurse in the hospital have been changing very
rapidly especially in the larger institution for e.g. intensive care, or the care of the acutely ill
patiently , is now more often given in a special unit where vital signs and other observations
are recorded y the machines. This is called monitoring and this term intensive care
unit(ICU)is becoming well known to all. New diagnostic test are being developed constantly
and many functions once routinely done by the nurse. This may be more gradual change in
more areas but, nevertheless, it is a change and it is bringing about a need for greater

specialization in nursing activities. So it is accurate to say that the hospital nursing services
can be both more comprehensive and more highly specialized than even before.
The possible position of the nurse in the hospital are given below,
Staff nurse:
Provide direct patient care to one patient or a group of patients. Assist in ward
management and supervision. Is directly responsible to the ward supervisor.
Part-time Staff nurses:
Same responsibility as a staff nurse but works less hours
Ward Sister / In-charge nurse / Supervisor:
Is responsible to the nursing superintendent for the nursing care and management of the
ward or unit. Takes full charge of the ward and assign works to nursing and non nursing
personnel working on the ward. Is responsible for safety and comfort of patient in the
ward and providing a teaching atmosphere if it is a teaching hospital.
Home sister / Warden:
Is responsible to nursing superintendent for the general care and welfare of those residing
in the nurses home. This includes counseling, recreation, diet and discipline as well as
supervision of the staff.
Department supervisor / Assistant Nursing superintendent:
Is responsible to Nursing superintendent and Deputy nursing superintendent for the
nursing care and management of the more than one ward or unit on a floor or within a
department e.g. department of operation theatre, out-patient or obstetrics.
Deputy Nursing Superintendent:
Is responsible to Nursing superintendent and assist in nursing administration of the
hospital officiate in the absence of Nursing superintendent.
Nursing Superintendent:
Is responsible to Medical superintendent for the safe and efficient management of
hospital nursing services.
Director of Nursing:
Is responsible to Medical superintendent for both nursing service and nursing education
within a teaching hospital.
If the hospital is large, there may be assistant or deputy positions to each of the above. If
the hospital is a teaching institution, responsibility of ward sister and supervisor may include
those teaching . This will not always be indicated by the title. In the same way, a Tutor may often
share administrative responsibility in nursing services although this is not indicated in the title.
Specific responsibility will vary with the institution.
Qualification for the nursing position in the hospital are variable because need for personnel is
often greater than the number of qualified personnel available.

It is important to choose a hospital for employment according to interests you have and the kind
of person wish to do. The clinical experience in a general hospital, for e.g. will be broader than
that in one offering more specialized services.
Hospital can be classified in a number of ways. If classified on the basis of control like,
1. Military hospital.
2. Railway hospital.
3. Government hospital.
4. Private hospital.
Each will have its respective policies and variety of services.
Although On duty time may still be very restricting and demanding in a number of
hospitals, an effort is being made to reduce the number of working hours and make the demand
of p[professional responsibility more reasonable. This again means that more nurses must be
employed.

Specialized Nursing Services in the Hospital:


Surgical Nursing Careers
As a surgical nurse, you will assist during delicate organ transplants, precision laser incisions and
quadruple heart bypasses, to name a few. From preparing patients before surgery to assisting the
surgeon in the operating room to charting progress in recovery, surgical nurses are there for
patients every step of the way. With a mastery of clinical skills and ability to connect with
people, as a surgical nurse you are an advocate for your patient during surgery. Monitoring vitals
signs, alleviating discomfort and comforting anxious patients and their families are all a
rewarding part of a career in surgical nursing.
Nurse anesthetists help prepare patients for surgery, give them anesthesia during surgery or
childbirth, and assist them through recovery.
Critical care (emergency room or acute/intensive care) nurses
are called on to watch over patients with the most critical health needs: either acute situations
requiring an emergency room visit or
chronic situations requiring intensive, cardiac, or highly complex care.
In seeking to improve the health of their patients and the work environment of their
staff, nursing administrators and managers dont only give care, they design it; they not only
follow policy, they make it; they dont just make do with the resources they have, they help to
allocate the resources.
Nurse-midwives deliver babies and help women to stay healthy and avoid complications during
and after pregnancy.

Perinatal (also known as neonatal or obstetric) nurses monitor and care for women before,
while, and after they deliver their babies.
Pediatric (children"s) nurses
care for children and adolescents: monitoring their growth and development, treating and trying
to prevent their illnesses, helping
them with asthma and other chronic problems, and working with them to overcome any
disabilities they might have.
Psychiatric and mental health nurses
assess and respond to people"s mental health needs, whether it be in a counselor"s office, a
hospital psych ward, or in an evacuation center during a natural disaster.
Hospice nurses
make the process of dying easier for terminal patients and their families. They provide emotional
support and whatever physical care is required to minimize patients pain and symptoms while
maximizing their dignity.
Further Education, training and experience sometimes requiring Master Degree or higher
Diploma can be helpful for Promoting at higher post.

2.Community Health Nursing services.:


Community Health Nursing is the term currently used to represent the original term tom
public health nursing. The health of general population become a major concern of the
government some years ago when it was found that facilities to meet the health needs of the
Indian rural population were most inadequate. An efforts to meet these needs began in 1951
with recommendations by the Bhore committee report. These recommendations were
implemented through five year plan.
The primary health canter, supplied with a medical officer and other related health personnel,
is the basic unit established in the rural area. Preventive medicine is the major aim of this work.
Services given are antenatal clinics, family planning, immunization and health teaching
regarding sanitation, prevention of disease and promotion of health.

Auxillary Nurse Midwives / Health Workers: They are commonly employed in the primary
health center to give maternal and child care. Professional nurses serve in these units as staff
nurses and supervisors. Their responsibilities and education when nursing students are also
present.

Community health nurse is also part of the services offered by the general hospital in
addition to then primary health centers. This has resulted from the criteria for recognition of
nursing g educational programmes which include experience in community Health Nursing. The
Government strongly supports. This and encourages its development through giving financial aid
to the individual institution. Many hospitals have well developed community health programmes
reaching out into the local community. All profession nurses now have basic training in both
community health nursing and family planning.
Community health Nursing is either of the above areas was not easily accepted,
understood nor respected by the public. Nursing personnel in this work had a most difficult time
in their early works. With continue efforts and support from the government, however, this has
changes greatly. Some of the original aims have been realized and many opportunities have
opened up for the professional nurse3.
Community Health Nurse attempt to keep entire communities as healthy as possible by
preventing and containing the spread of the diseases and the incidence of environment related to
health problems, as well as, by educating people how to maintain their wellness . They are
employed by communityorganization ranging in size from camps and schools to city health
department.

3.Teaching in Nursing:
Professional nurses who are interested in teaching have broad scope. Teachers in are
needed in all areas of clinical practice and theory. The major area of clinical practice are of Mid
wifery, community Health, Pediatric and Medical-Surgical Nursing. Other less common but
developing areas are those of i9ntensive care, coronary care, ophthalmic and psychiatric nursing.
The function and responsibility of teacher in nursing will vary with the institution. All
teachers will have responsibility for planning, teaching, and supervising. Learning experiences
for the student; acting as advisor to the student and keeping upto date with what must be taught.
In many institution, the nurse in nursing education will share administrative responsibilities with
those in nursing service in the hospital; usually departmental supervisor. This has developed as
an effort to bring theory and practice closer together. In other institution, only teaching activities
will be expected of those in nursing education. The nurse who work as a dean, or director of
nursing education, usually caries only teaching and administrative responsibilities in education.
Nursing education is inn a process of change at the present time. Professional nurse are
qualified to teach by taking advanced courses which specialize in area of interest. For some
years, the most common qualification was tutor course, but now Bachelors degree and
experience is preferred. The current transition being made in basic programme of nursing
education may eventually cause further change in these qualification.

Qualification for teaching will depend upon the type of educational programme in which
one will be working. A graduate of tutor course may be qualified to teach student s in an ANM/
HW programme. Bachelors degree and more is require to qualify to teach certificate/Diploma
programme and Bachelors degree in nursing. Nurse with a Masters degree qualify to teach
students in Masters degre4e nursing programme. Certain teaching on Masters degree level is
requires education to level of the Doctorate degree.
Position in nursing education may overlap with the same level of administrative position in a
hospital. This means a supervisor who practice in a hospital may also employed as an instructor
by the Nursing School in that same specialty.

Clinical Instructor: ( B.Sc.Nursing, P.B.B.Sc.Nursing or M.Sc.Nursing)


Is responsible to the Principal or Vice-Principal or Nursing Superintendent for
supervision and teaching of student in Public Health Nursing /CHN. Among other
responsibility are those of being responsible for planning assignment of patient s
maintaining records and helping student with extra curricular activities.
Lecturer: M.Sc.Nursing
He/ She works under the direction of the departmental head and assist him in
administration, instruction and guidance activities.
Asssistant Professor: M.Sc.Nursing with experience / PhD in Nursing (any speciallity)
Usually works under the professor and HOD of the particular department of speciality
and assist him in administration, teaching, guidance and counseling and research
activities.
Professor: M.Sc.Nursing with experience / PhD in Nursing (any speciallity)
The Professor is over all in-charge of the department and thereby responsible for
administration, teaching activity and guidance of that particular department.
Vice-Principal / Principal: M.Sc.Nursing with experience / PhD in Nursing (any
speciality)
Vice-principal works under the Principal and helps in administration and guidance of
staff.
Principal is administrative head of School/ College will be directly responsible to
Director of Medical Education / Director of Health and Family welfare services and
responsible for implementation and revision of curriculum for various courses and
research activities of school/ college.

The professional nurse who teaches will find the work stimulating and satisfying and also
demanding.

4. Industrial Nursing:

Nursing services in industry have developed rapidly as more and more large industrial
companies have developed in the country. The factory Act of 1948 made it legally necessary to
maintain definite standard of health, safety and general welfare for factory /Industrial workers.
Enforcement of these law has brought about the need for both Medical officer and Nurses.
Services to industrial workers varies from simple first Aid procedure in a very basic clinic
environment to specialist service in well equipped hospital.
The nurses working in occupation / Industrial areas are concerned with health and safety
of employees in the workplace. IN normal situation they try to help workers stay well and
productive and prevent the types of ergonomic problems that lead to stress injuries. When
accident occur, they make sure that employee get the Medical attention they need and that
workers compensation, if necessary, is dealt with correctly.
Function of industrial; nurse and hours of duty in industrial hospital will be similar to
those in most general hospital. So the professional nurse in industry is usually highly paid and
has convenient working hours.

5. Military Nursing:
Military nursing was the earliest type of organized nursing in India. Although it was
begun by services of British army nurses it has developed into a well organized service givien by
the Indian International Nurses only. The temporary Military nursing service, formally organized
in 1914. Begun with few nurses but gave the foundation for rapid development of Military
nursing services which is known today. Nursing in military was unpopular at first because of the
unknown and isolated areas in which this service had to be given.
A permanent Nursing service was formed after World war I. After that the military
nursing service became a part of Indian Army by which means nurses became commissioned
officers who were rank from Lieutenant to Brigadier.
The military nursing service also began to develop their own nursing educational
Programmes like GNM and Degree programme. Arrangement are now made to utilize both
married and un married nurses with the added possibility of civilian nurses serving under
deputation and lien on civilian appointments when contracts are completed.
Military services offer attractive economic and security benefits. Salaries are very high
compared to those in other nursing services. Opportunities are also given for advance study
which the allows for increased special allowances. Opportunity to study abroad is common. The
function of nurse in military service vary from direct care in bed side nursing to teaching,
supervision and administration depending upon qualification and rank.
To work in military nursing you should have real sense of courage and willingness and
also capacity of extra-ordinary devotion. You should give serious consideration to service in

Military Nursing Service when choosing a career. Experience is vast and professional
opportunities for personal development, continuing education and promotion are readily
available.

6. Private Duty Nursing:


There are times when individual patient either desires or needs a nurse for his own care alone.
This happens when,
- Patient is acutely ill and health needs cannot be meet by general nursing personnel only.
- Patient is convalescing at home without available help.
- Patient has terminal illness.
Private duty nursing is not widely practices because of very high cost to patient. It is restricted to
wealthy patient.
Larger hospitals and private nursing homes, however, do have individual registers of
nurse available to do private duty when it I needed. This area of nursing service which should be
considered by the professional nurse.
The regular hours and choice of patients and working hours make it convenient type of
work for the nurse who may have other personnel responsibilities. It is also common for retired
nurses to work in this area the retire nurse is valuable in this service because their broad
professional knowledge and experience.

7. Nursing in Red Cross Society:


Indian Red Cross Society was established in 1920 with major aims of helping others
from a neutral standpoint. It is most active in giving relief to needy and suffering people at times
of major disasters and in times of war.
An organized section of Red Cross Society was developed for nursing services in 1959.
Nursing service activities are centered upon teaching home nursing to lay people. Nurses who
are interested can take a course and become a Instructor in Home Nursing. More recently,
professional nurse are also being used to teach practical nursing to Non-professional personnel in
hospitals. This is called the Volunteer Nurses aid services.
Professional nurses can practice in Red Cross Society in a number of ways. The two most
common services are those of giving direct care to sick or wounded and teaching of lay people.

8. Travel Nursing Careers

Travel nursing lets you be in control of your nursing career. You choose the location, specialty
and length of commitment for each nursing assignment. With a shortage of qualified nurses in
hospitals and clinics across the country, you can find short-term work (typically eight weeks or
as long as 26 weeks) in virtually any location and offering generous compensation. Many

There are a number of ways in which nurse can work abroad. If nurse is qualified for a
higher level of administration then she sent for short period of observation and experience by the
organization for which you work.
Nurses can work abroad as immigrant which means to go abroad on a visa which allows ,
an indefinite length of stay. In this arrangement it is necessary to apply for registration in foreign
country either on the a stat5e or national level. The receiving country will evaluate your
educational preparation to decide eligibil;ity may demand an affiliation with practical experience
in a certain clinical areas such as psychiatric nursing, or you may be asked to take one or more
written examinations.
Nursing services abroad can also be done bon a contract basis. Countries like Arabia,
Wext Germany and S. Africa recruits foreign nursew to serve for two or three years at a time.
Many nurses go abroad for this type of services.

13. School nurses & College nurses


School nurses help kids in away-from-home schools. They make sure children are immunized,
give eye and ear tests to make sure that any problems with those faculties are detected and
corrected, treat injuries and illnesses that occur at school, teach and counsel kids about their
health, and sometimes provide daily assistance to "mainstreamed" children with severe medical
challenges.
College nurses
assist older young people and adults at colleges and universities. They too treat illnesses and
injuries, but they typically do more counseling and education to try to prevent psychological
problems, alcohol and drug abuse, unintended pregnancy, sexually transmitted disease, and other
health concerns that occur more often at college campuses.

stay well and treating injuries if they occur.

with health care providers and offering health advices. Nurses use assessment and therapeutic

Companies that sell product such as pharmaceuticals, I.V.equipments and monitors often hire
professional nurses as sale representative. Nurse provide credibility for product promotion,
especially when they have used the peroduct in clinical practice.
Private Consulting:
Health care organization use consulting firms to prepare for accreditation visits, apply for
quality wards meet employees educational needs, engage in work design and look for ways to
reduce operating costs.
The successs of nursing consultationdepend upon on personal proficiency in the area where
consultation is provided; strong therotical, business and clinical skills, the ability to solve
problems quickly and ability to compete with other consultant.

16. Nursing services in other areas


Nursing researchers/scientists
research and evaluate all aspects of the nursing practice, including the effectiveness of nursing
tools, methods, and procedures.
More recently there are occasional opportunities to do these things on a full time basis either
temporarily or permanently.
Services in the areas or research and writing requires higher education. Professional nurses
qualified to do this work are greatly needed in India.
Nurses who are interested in organizational work can also work in nursing organization like
position as a fulltime secretaries on state level with the TNAI are available for experiences
nurses. Secretaries of other professional organization are also often employed on full time basis.
This kind of work usually requires broad experience in the profession and ability to write.
Nursing service administrative positions on state level, some of which were onced filled
by medical personnel are now being filled by professional nurses. The highest
administrative position on a national level is that of Nursing Advisor to the Government
of India.
Higher education and broad experience is required for this position.
