GUIDELINES IN PERIODICAL EXAMINATIONS

Periodical Examinations with their Table of Specifications that are prepared by instructors should based its contents to the
course syllabus/instructors guide being given to them by the curriculum developer in their subjects.
Prepared exam should be validated by the respected deans of the department prior to the collection of the said examination.
All periodical examinations should be passed to the testing center a week prior to the said examination for analysis,
reshuffling/rephrasing to prevent leakage of the said exam.
All examinations that are collected will be reencoded and be put on a test bank that will a basis for the next examinations.

During examinations, rules written at the back of the test booklet should be strictly implemented by the proctors and observed
by the students.
Students who are caught having a leakage at hand will not be able to continue his/her said examination and will have
automatically a failing grade for the said examination.
Students who are caught cheating or having a leakage shall leave the examination room.
No students should be late more than 15 mins prior to the start of the said examination.
Students who comes too late for the said examination will not be able to take the exam on the examination day.
Examinations will be collected and arranged by testing center personnels.
Examination booklets will be released 24 hours after the said examination.
Students who are not able to take the exam shall present their excuse letter to the testing center coordinator ,for those who had a valid
reason can take the examination without charge and 500 pesos to those students that take their examination by chance, per subject at the
cashier.
Special exam are given to students after paying the exam fee at the cashiers office.
The testing center will conduct a random items analysis one section per subject to validate / evaluate students level of difficulty in answering
the questions.
Those questions that are very hard for them to answer will be subject for change , and instructor will be informed about the
results/interpretation and analysis of the examination given.
