OBJECTIVE
The objective of the XTREME POWER FUELS ALLIANCE INC. Emergency Action
Plan is to:
B. Comply with the Occupational Safety and Health Center Rules and Regulations of the
Department of Labor and Employment
C. To prepare employees reporting to the Company for dealing with emergency
situations.
D. To minimize injury and loss of human life and company resources by training
employees, procuring and maintaining necessary equipment, and assigning
responsibilities.
This plan applies to all emergencies that may reasonably be expected to occur at SITIO
UPPER MANALITE 2, BRGY STA CRUZ, ANTIPOLO CITY, RIZAL (Site).
i.
BACKGROUND
XTREME POWER FUELS ALLIANCE INC. is a 100% Filipino owned Corporation and
is an authorized dealer of Petron Fuels, Lubricants, and Liquefied Petroleum Gas. Its
location is along Marcos Highway at Sitio Upper Manalite 2, Barangay Sta. Cruz,
Antipolo City, Province of Rizal.
The Company was incorporated on May 13, 2014 and assumed operations of the Petron
Cogeo Service Station on June 17, 2014. It currently employs 3 individuals while 17
contractual employees from various employment agencies report for work in the site.
The features of the site include two (2) canopies with 4 filling points each for customers,
9 underground tanks for fuel storage, an open space for a potential convenience store, and
a service bay for potential mechanical services.
While sale of the aforementioned products is the only operation the company has
currently, it is exploring other related ventures in the Site as well.

SCOPE
PLAN IMPLEMENTATION
Under no circumstances shall an employee attempt to fight a fire that has passed the
incipient stage (that which can be put out with a fire extinguisher), nor shall any
employee attempt to enter a burning building to conduct search and rescue. These actions
shall be left to emergency services professionals who have the necessary training,
equipment, and experience (such as the fire department or emergency medical
professionals). Untrained individuals may endanger themselves and/or those they are
trying to rescue.
L. Informing XTREME POWER FUELS ALLIANCE INC. Employees of Fires and
Emergency Situations
In the event of a fire or emergency situation, ASM shall ensure that all employees are
notified as soon as possible using the building alarm system (which includes both audible
and visual alarms 24 hours a day). ASM shall provide special instructions to all
employees via the public address system.
If a fire or emergency situation occurs after normal business hours, ASM shall contact all
employees not on shift of future work status, depending on the nature of the situation.
M. Corporate Notification
1. ASM shall contact Management as soon as possible if media coverage of the
situation is expected.
2. ASM shall contact Management as soon as possible with information on
employee injuries and/or loss of life, property damages, theft, or cargo
losses.
N. Emergency Contact Information
ASM shall maintain a list of all employees personal emergency contact information and
shall keep the list in an easily accessible area in the event of an emergency.

All individuals remaining behind to shut down critical systems or utilities shall be
capable of recognizing when to abandon the operation or task. Once the property and/or
equipment has been secured, or the situation becomes too dangerous to remain, these
individuals shall exit the building by the nearest escape route as soon as possible and
meet the remainder of the employees at the Designated Assembly Area.
P. Advanced Medical Care
Under no circumstances shall an employee provide advanced medical care and treatment.
These situations shall be left to emergency services professionals. Untrained individuals
may endanger themselves and/or those they are trying to assist.
Q. Accounting for Employees After Evacuation
Once an evacuation has occurred, ASM shall account for each employee/visitor assigned
to them at the Designated Assembly Area. Each employee is responsible for reporting to
the appropriate ASM so an accurate head count can be made. All employee counts shall
then be reported to the Emergency Action Plan Manager as soon as possible.
R. Re-entry
Once the building has been evacuated, no one shall re-enter the building for any reason,
except for designated and properly trained rescue personnel (such as fire department or
emergency medical professionals). Untrained individuals may endanger themselves
and/or those they are trying to rescue.
All employees shall remain at the Designated Assembly Area until the fire department or
other emergency response agency notifies ASM that either:
1. the building is safe for re-entry, in which case personnel shall return to their
workstations; or
2. the building/assembly area is not safe, in which case personnel shall be
instructed by ASM on how/when to vacate the premises.
S. Severe Weather
The ASM shall announce severe weather alerts (such as typhoons) via SMS. All
employees shall immediately retreat to the Designated Area until the threat of severe
weather has passed as communicated by the ASM.

This Emergency Action Plan shall be effective June 1, 2016 and will be reviewed
annually, or as needed if changes to the worksite are made, by ASM. Following each fire
drill, Management and ASM shall evaluate the drill for effectiveness and weaknesses in
the plan, and shall implement changes to improve it
