JUDICIAL AFFIDAVIT
OF MR. SANTIAGO S. SANTIAGO

I, SANTIAGO S. SANTIAGO, 45 years old, single , residing at 14

Lubang St., Ayala Alabang Village, after having been sworn to in accordance
with the law do hereby depose and state:
That Atty. Zari Charisamor V. Zapatos is the counsel who conducted
and supervised my examination as a witness at her office at Mango bldg., 7 th
flr, rm 505, Muntinlupa City; That I am answering the questions herein fully
conscious that I do so under oath and that I may be criminally liable for false
testimony or perjury;
The following are the Questions propounded by Atty. Zari Charisamor
V. Zapatos and my answers in the English language:

Q1:

Do you swear to tell the truth and nothing but the truth?

A1:

Yes Maam.

Q2:

Are you Santiago S. Santiago? One of the subscribing witnesses to

Pedro Kalambutans will?

A2:

Yes Maam

Q3: How was the name of Pedro Kalambutan signed on his will?
A3: Pedro politely asked me to use a rubber stamp that was in the form of
his signature. Thats what I used to sign his name on the parts of the will
that required his signature.
Q4: Where were you at the time the will was being signed by your fellow
witnesses? Where was Pedro?
A4: We were in his home in Katarungan Village, Muntinlupa. I was sitting
with Pedro, Rosa, at the round table in Pedros dining area. Anton stood very
briefly to make us coffee.
Q5: Was every page of the will, except the last, signed by you, rosa, and
anton on the left margin? Were you also the one who stamped Pedros
signature on the margins of the will?
A5: Yes. And yes, I was also the one who stamped Pedros signature on the
margins.
Q6: Did you, pedro, rosa, and anton acknowledge the will before a notary
public?
A6: Yes we did.
Q7: What do you think was pedros mental state when you were executing
his will?
A7: He was perfectly sane during the time we signed the will.
