This Judicial Affidavit of Maria Veronica Primitiva Duterte (A.K.A.Estrella S. Alfon) is executed
to serve as her direct testimony in the instant case.
This Judicial Affidavit is being offered to prove:
a) All allegations in the Petition including all annexes appended thereto and which were already
marked as exhibits during the Pre-trial of the case;
b) All other related matters, facts and circumstances relevant and material to this case.
This Judicial Affidavit was taken at the office of Atty. Krizzy Kalerqui at Philamlife Tower 122
Salcedo Village, Alaminos City.
Questions were propounded by Atty. Krizzy Kalerqui and these questions are numbered
consecutively and each questioned is followed by the answer of the witness.

1. Do you swear to tell the truth and nothing but the truth?
A: I swear.
2. Are you aware that of the criminal charges that may be charged against you for fase
testimony and perjury if you will not tell the truth?
A: I do.

3. Please state your name and personal circumstances.

A: I am, Maria Veronica Primitiva Duterte (A.K.A.Estrella S. Alfon) , 25 years old,
single, Filipino, residing at #12 Kapayapaan Street, Bangkal, Alaminos City, Pangasinan,

A: I was born on February 2, 1991.
5. Where were you born?
A: I was born at Lourdes Hospital, Alaminos City.
6. What name is reflected on your birth certificate?
A: The name that is indicated on my birth certificate is Maria Veronica Primitiva Duterte.

7. By what name were you known when you were a child?

A: I was known as Estrella S. Alfon since I was a child.
9. What are the names of your parents?
A: Their names are Filomeno Duterte and Estrella Veronica Primitiva Duterte.
10. Were they the ones who took care of you and raised you?
A: No, maam.
11. Who raised you?
A: I was raised by my uncle, Mr. Hector Alfon, whom I call Daddy and whom I
consider as my father.
12. Why did Mr. Hector Alfon raise you?
A: Because my parents abandoned when I was an infant and was left in the custody of my
uncle, Mr. Hector Alfon.
13. Do you keep in touch with your parents?
A: No they have not exterted any effort in reaching out to me.
14. Who provided for your schooling?
A: My uncle, Mr. Hector Alfon, provided for my schooling.
15. What name is reflected on your school records?
A: The name that is reflected is Estrella S. Alfon.
16. How long have you been a resident of your community?
A: I have lived here, as far as I can recall, when I was a child.
17. What name do they call you in your place of residence?
A: They call me Estrella S. Alfon or Estrella, for short.
18. What name is reflected on your college diploma?
A: The name that is reflected is Estrella Alfon.
19. What name is your reflected on your Voters ID and other government-issued identification
cards?
A: The name that is reflected is Estrella Alfon.

A: Yes, Atty.

19. By what name are you known in your workplace?

A: I am known as Estrella S. Alfon.
20. Why do you want to change your name?

A: Because I have been known since I was a child as Estrella S. Alfon, and to avoid
confusion since all my school records, government issued identification cards and other proof
identity all reflect that my name is Estrella S. Alfon.
