Mr. H. is a 42 year old gentleman with known coronary artery disease who presented to the
emergency room with angina on June 9. He previously had a myocardial infarction in January of
1994 and under went thrombolytic therapy with TPA. His course was complicated by postinfarction
pericarditis and cardiac catheterization revealed left ventricular dysfunction, minimal nonobstructive
disease in the coronary system and total thrombotic occlusion of the right coronary artery. Today, the
day of admission, he had again sudden onset of squeezing chest discomfort typical of his previous
MI pain in 1994. He therefore immediately presented himself to the emergency room where they did
