Work Productivity and Activity Impairment Questionnaire:

Specific Health Problem V2.0 (WPAI:SHP)*


The following questions ask about the effect of your PROBLEM on your ability to work and perform
regular activities. Please fill in the blanks or circle a number, as indicated.
1) Are you currently employed (working for pay)?
If NO, check "NO" and skip to question 6.

2) During the past seven days, how many hours did you miss from work because of
problems associated with your PROBLEM? Include hours you missed on sick days, times you went in
late, left early, etc., because of PROBLEM. Do not include time you missed to participate in this
study.

Think about days you were limited in the amount or kind of work you could do, days you
accomplished less than you would like, or days you could not do your work as carefully as usual. If
PROBLEM affected your work only a little, choose a low number. Choose a high number if
PROBLEM affected your work a great deal.

activities, other than work at a job?
By regular activities, we mean the usual activities you do, such as work around the house, shopping,
child care, exercising, studying, etc. Think about times you were limited in the amount or kind of
activities you could do and times you accomplished less than you would like. If PROBLEM affected
your activities only a little, choose a low number. Choose a high number if PROBLEM affected your
activities a great deal.

Recent analyses suggest that the accuracy of responses to questions 5 and 6 by some subjects
in the WPAI:SHP might be improved by repeating the instructions directly above the scales.
Version 2.0 has these instructions, but in all other respects is the same as the original
WPAI:SHP.
To adapt the WPAI to a specific disease/ condition, replace PROBLEM throughout with the
name of the disease/ condition. To use the WPAI in clinical practice, and not as part of a
study or clinical trial, use the Clinical Practice Version (CPV) which omits reference to
participation in the study in questions 2 and 3.

The WPAI yeilds four types of scores:


1. Absenteeism (work time missed)
2. Presenteesism (impairment at work / reduced on-the-job effectiveness)
3. Work productivty loss (overall work impairment / absenteeism plus presenteeism)
4. Activity Impairment

The sum of specific health problem impairment and impairment due to other health reasons is equal to
impairment due to all health reasons. WPAI outcomes are expressed as impairment percentages, with
higher numbers indicating greater impairment and less productivity, i.e., worse outcomes, as follows:

Multiply scores by 100 to express in percentages.
