Delivery Aid to Women with Mild Preeclampsia

In one morning, at Permata Husada Independent Practice Midwife (IPM), a man brought
his pregnant wife. That place was not quite far from their house and his wife did the antenatal
care regularly at Permata Husada IPM.
They came to check the wifes condition since the wife has got contractions and blood
slime discharge in the morning.
Mr. Didi

: Assalamualaikum, Ms. Yasmin.

Ms. Yasmin

: Waalaikumsalam wr wb. Hi Mr. Didi, Please come in

Ms. Yasmin

: Have a seat. What brings you here?

Mrs. Janet

: Since in the morning, Ive got contractions and blood slime discharge. I feel
languid today miss.

Ms. Yasmin

: Okay, I will do some examination later, but first, do you bring your Pink book?

Mrs. Janet

: Yes, I do. Here

Ms. Yasmin

: Please follow me to the examination room, Mrs. Janet


After 15 minutes, the midwife and obstetrician did some anamnesis and complete
examinations.

Mr. Didi

: Hows my wife condition, doc?

Ms. Intan

: This is your wifes first pregnancy. Now, your wife is in first labor stage. The
contraction is normal, and shes having blood slime discharge which is one of the
sign of early labor stage. But, the problem is her blood pressure is pretty high. It

is 140/90 mmHg and theres protein in your urine. Do you have a history of high
blood pressure before youve got pregnant or in your family?
Mrs. Janet

: From my family and my husbands family, no one of them had a high blood
pressure, doc. So, hows my baby condition, mam?

Ms. Intan

: It will be dangerous, so we will give an action immediately. But, you dont have
to worry, your babys condition is fine, the position is normal, its heart rate is 140
times/minute, which is normal. Just relax, if you feel worry so much, it will gain
your blood pressure and it will put your baby in dangerous condition. We will
keep observe you and your baby condition.

Mrs. Janet

: But, Im so scared if something happens to my baby.

Ms Intan

: Its okay. Ms. Yasmin and Ms. Nisa will lead your labor. Your husband will be
around you all the time. All you have to do is keep praying and follow the
midwives instructions

Mrs. Janet

: Okay, doc.

Ms. Intan

: For now, we will get you the IV line since you feel languid.
Ms Yasmin and Ms. Nisa kept observing Mrs. Janet condition. Ms. Nisa did the
preparations of labor. After 6 hours, cervical dilatation had completed and
amnion membranes had ruptured. The midwives wore all the self-protections
tools and lead the labor.

Ms. Nisa

: Okay, Mrs. Janet, its the time. Your baby will come out soon, keep praying and
follow my instructions. Mr. Didi, please stand beside her and keep encourage her.
If she feels thirsty, she may have a drink

Mr. Didi

: Okay, maam.

Ms. Nisa

: Mrs. Janet, your hands have to hold the feet and stick your chin to the chest,
okay? You can bite a towel. It will help you to push. Dont close your eyes. If the
contractions gone a while you can breathe normally

Ms. Nisa

: Yes, you can push right now. Good! Keep pushing. Take a deep breath and push
hardly.

After 30 minutes, the baby was born in normal condition. The placenta was delivered 5
minutes after.
Ms. Yasmin

: Congratulations for both of you! Its a boy. His weight is 3600 g and height is
50 cm. I already gave him vitamin K injection to prevent brain hemorrhagic.

Mrs. Janet

: Thank you so much, Maam.

Ms. Nisa

: Youre welcomed. Well keep observing both of you and your baby conditions.
Well make sure that you and your baby conditions are good, so then you can
return home very soon.

Mrs. Janet

: Okay, Maam.

Ms. Yasmin

: Mrs. Janet, later the intern will come here to check your condition and they will
give you all the information that you need. Is it okay?

Mrs. Janet

: Its okay, maam. Id love to.

After 1 hours, Zunda and Dita as the intern checked the mother and baby condition.
Ms. Dita

: Hi, Mrs. Janet. How do you feel right now?

Mrs. Janet

: Im fine, Miss. But, I feel so hungry.

Ms. Dita

: Yes, its normal, because the labor was draining your energy. First, let me check
your blood pressure.

Ms. Dita

: Its still 140/90 mmHg. You should take a rest and the important thing is you
should have a meal now, theres nothing should not be eat, unless you have an
allergic to something. You must avoid salty, spicy and greasy food. For drink you
must avoid caffeine, all kind of carbonate drink and alcohol, because it can raise
your blood pressure. Do you understand, Mrs. Janet?

Mrs. Janet

: I understand, Miss.

Ms. Dita

: Okay, I excuse myself. My friend will check your baby conditions, bye.

Mrs. Janet

: Bye bye.

Five minutes later, Ms. Zunda came to Mrs. Janets room to checked her babys
conditions.
Ms. Zunda

: Excuse me, Mrs Janet, Im Zunda. Im here to check your baby condition.

Mrs. Janet

: Of course. I have so much to ask you, can my baby get bath now?

Ms. Zunda

: I dont think so Miss. The baby can get bath after 6-24 hours after labor to
prevent hypothermia, so you have to make sure that your baby is always in warm
condition. For the umbilical cord treatment is you have to wrap it with sterile
bandage every time its get wet and do not put anything on or in the umbilical
cord such as alcohol, coffee, baby oil etc.

Mrs. Janet

: Thats good information, thank you. How about the right position to breastfeeding the baby?

Ms. Zunda

: Okay. First, you have to clean up your breast and nipple area with warm water,
than sit straight make sure the babys chest and belly adhering on your stomach.

The nipple and areola must be in the babys mouth. Watch the babys nose so he
can still breathing.
Mrs. Janet

: I see. Can I give formula milk?

Ms. Zunda

: No, Im afraid you cant. You must breast feed your baby exclusively till 6
months, after 6 months you can give the mixed feeding till he is 2 years old.
Make sure he gets his breast feeding exclusively for 6 months because there is so
much benefit from it. It doesnt need cost, it help your recovery after labor, rich
of nutritions, the baby will get immunity and more intelligent in many ways

Mrs. Janet

: Ooh, I know now. I will follow all of your instructions at home.

Ms. Zunda

: Mr. Didi, you have to keep support your wife, a fathers role is important in
taking care of baby

Mr. Didi

: Yes, maam. Ill do anything to take care of my family.

Six hours later, Mrs. Janet and her family was allowed to return home.
Mrs. Janet

: Thank you so much for the doctor and the midwives, thank you for the kindness
and the treatment.

Ms. Intan

: Its a pleasure to help you, Mrs. Janet. If you have any problem after labor,
please come again. We welcomed you guys anytime.

Mrs Janet

: Of course. Thank you. Bye bye.

Ms. Nisa

: Bye, take care.
