a. Approved seagoing service on board of at least two months

DOCUMENTARY REQUIREMENTS
FOR THE REVALIDATION OF CERTIFICATE OF PROFICIENCY
1. Certificate of Sea service from Manning/Shipping Company (in MARINA
prescribed form)
a. Approved seagoing service on board a particular vessel of at least twelve
months within the last five (5) years or function to be considered equivalent to
seagoing service, (please see 4.18 of STCW Circular No. 06 Series of 2014.),
depending on the COP that the seafarer is applying for.
b. Certified True Copy of filled-up SIRB pages showing seagoing service
2. Photocopy of 1st page of Seafarers Identification Record Book (SIRB)
3. Original certificate to be revalidated
4. Affidavit of loss In case of lost or damaged certificate
5. For SSO COP, acquired before January 1, 2012, the seafarer is required to
present Certificate of Completion on Anti-Piracy and Anti-Armed Roberry
