INTRODUCTION
This report is intended to give the reader an overview of how ABP manage the
navigational safety of the Humber Estuary, Rivers Ouse, Trent and the Humber, in
relation to its obligations as a Harbour and Pilotage Authority. The Humber continues to
be the busiest estuary in the United Kingdom with some 13,070 inward shipping
movements in 2011, giving rise to 28,708 vessel movements in total. In addition, there
were 4,259 recorded Oil Barge movements moving 1.35 million tonnes of product
between Immingham, Hull and upriver destinations. Latest available statistical figures
indicate that in excess of 76.9 million tonnes of cargo and over 1.007 million passengers
were handled.
An order for a new pilot launch was placed with Mustang Marine of South Wales in May
2010. The Nelson 48/50 launch is from the same mould used for our existing fleet but
previously supplied by Halmatic. The boat was delivered in early 2011 and named
Humber Mercury.
Towards the end of 2010 Siemens and Associated British Ports announced that they
have signed a Memorandum of Understanding in connection with ABP's proposed
Green Port Hull development at Alexandra Dock, Hull. The parties have agreed to work
together to develop plans for the construction of a new Siemens offshore wind turbine
manufacturing and export facility at the port with the aim of executing definitive
agreements in 2012.
On the South bank, Able UK are also proceeding with a consent application to build a
berth between South Killingholme Oil Jetty and Killingholme Haven to support offshore
wind farm manufacturing and exports.
Consent applications for the building of an outer Ro-Ro berth at Grimsby and the
deepening of the Sunk Dredged Channel were also submitted in 2010 and both were
approved early in 2011.

2011 saw a slight decrease in river traffic overall (1.8% lower than 2010) due to the
economy, with highest concentrations of vessel traffic again centred on the Immingham
area. The number of reported Ship Defects accounts for the 8.5% rise in reported
Navigational incidents; we continue to work closely with the MCA and MAIB by passing
on these issues which result in targeted port state inspections.

Vessel Traffic Services Operator at the time of the incident/occurrence. As such, they have not been revised for

The year-end review of pilotage showed that on average a VLS pilot conducted 0.75 pilotage
acts per duty day and a non-VLS pilot averaged 0.93 acts per duty day.

number for the year 2010 was 17,368.
The total number of PEC passages, other than barge PEC holders, carried out in

Pilot boards vessel at Spurn Light Float and reports a defect with the ship's windlass. An electrical fault means that the vessel
can drop an anchor in an emergency but would be unable to recover it. Pilot and Pilotage Operations Manager (POM)
consulted. Vessel not to proceed to Flixborough at this time. Agent (Flixborough Wharf) contacted. River Side Quay (RSQ) or
Albert Dock has been suggested and agreed as a safe berth to have an engineer attend to fix the problem prior to completing
her passage. KGD contacted to request berth.
1845 RSQ approved but Assistant Dock Master (ADM) says no heavy gear would be allowed onto RSQ, and if the vessel went into
dock she'd be unable to move to Flixborough after repair on one tide.
1900 Agent informed. Attempting to organise a Dean's tug to escort the vessel all the way to Flixborough. Pilot and POM in
agreement providing defect is rectified before vessel leaves Flixborough.
1923 From ADM a dean's tug is penning out of KGD at 2000 to escort vessel to her berth at Flixborough. (Tug Shovette).

At approximately 1310 the Tor Fionia performed an overtaking manoeuvre of the
Flintercarrier and created a close quarters situation with the outbound vessel
Pagola. Pilot Taylor onboard the Pagola wanted to express his concern with the situation.
The manoeuvre commenced after rounding the 4A buoy with the Flintercarrier remaining to the north of the channel
whilst the Tor Fionia used the centre of the channel. The outbound vessel Pagola was on the south side of the
channel.
The Tor Fionia passed the Flintercarrier in the vicinity of the South Shoal and the proceeded on a NE'ly course and
passed the Pagola in the vicinity of Lower Burcom at close proximity and at high speed.

resume her passage downriver to sea. There was no reported damage to the vessel.

Vessel Ocean Endeavour outward bound from ALB Dock currently passing Paull Sand. Pilot calls VTS
on the telephone to report the vessel grounded whilst leaving the lock to the north. Vessel was
refloated quickly with no damage, pollution or injury to personnel.
Vessel departed Albert Dock at 2358 using spring placed on the last bollard of knuckle end. The vessel
was kept close to the bull nose on the stbd side and the spring was let go, using the bow thruster to
starboard. The vessel grounded on the port side aft. The tug ' Lashette' was used to pull the vessel
clear, passage was resumed at 0035. No damage was reported.

Reordered with 1 Deans tug support for 0800 24/1/11.
The pilot boarded at Blacktoft, where the vessel was starboard side to, stern to tide. At this time, the
hatches were not secured, the stbd anchor was tripped (flukes out) and there were a number of
vessel movements in and out of the Trent and Ouse. For these reasons, the pilot suggested that the
sailing be delayed until daylight. However, the Captain was keen to proceed, so this was agreed.
As the vessel sprung off the berth, the tide caught the stern and the pilot was unable to counteract.
The stbd anchor was let go as the stern took ground. Vessel was refloated at High Water using the
anchor to winch vessel into river. Pilot reports that the vessel was heavily underpowered. No damage
was reported to the vessel.

Captain reports vessel aground midway between East Middle Buoy and the Anson on departure from
RT1. Tug Svitzer Moira already requested from KGD to assist. Captain does not require any other
assistance as yet. Tide falling KGD gauge 2.08m. Coastguard informed. ( 789 passengers, 137 crew ).
2041 Captain reports he is underway again without tug assistance, and states he was half a cable
inside the buoy line when aground. Vessel checking for damage or passenger injury. Coastguard
informed. H.M informed. 2049 Captain confirms all is well onboard, vessel resuming passage.

Vessel left berth at 1.5 hours before LW. The vessel made way towards the East Middle Buoy to avoid
set towards the Hebbles shoal. When past the East Middle buoy, the vessel grounded in the navigable
channel, 0.5 cables inside the buoy line. At the time, the vessel was making a good rate of turn to port
but, probably due to interaction with the shoal, the vessel stopped. VTS was informed and assistance
requested from one Svitzer tug. After 6-7 minutes, working with engines, rudder and bow thruster, the
vessel refloated and continued passage. Sounding were taken of all tanks / voids and found to be ok. A
diver has been ordered for next arrival to confirm.

aground mid river, engines running, pilot standing by with boatman attempting to talk vessel back alongside when she has water. Pilot
will keep us updated. Vessel has anchor down. Inward vessel for Keadby warned.
2130 Pilot reports tide too strong at the moment to manoeuvre back to the berth.
2205 Pilot reports the Master is not following his advice with regard to anchors or manoeuvring, and has now run aground on the
opposite bank. The pilot will try to advise the Master further but is unable to see too much. RMS Vindava inward for Keadby proceeding
with caution and may abort berthing.
2218 Vessel now afloat and coming back alongside. Pilot will board and
investigate what has happened but will cancel sailing for this tide.
2230 Pilot reports Captain appears drunk. Duty Manager informed. Police will be contacted. Pilot from RMS Vindava will also be
attending Celtic Mariner to assist. Pilot reports that the anchor may have damaged the forepeak and will require inspection prior to
sailing. ( Bunkers 49.5 tonnes Diesel Oil, 2240 litres Lub Oil, 8 POB, Cargo of Broken Glass). Pilot reports no visible damage or
pollution but has concerns about the anchor which may have damaged the forepeak. Pilot recommends that the anchor remain as it is
at this time.

V/l briefly touched bottom close north of the Hawk anchorage. Whilst awaiting the tide to turn to return to Hull Marina the vessel briefly
touched bottom in a position with a charted depth of 3.5m.V/l was navigating in an area that has numerous shoal patches. V/l failed to
allow sufficient margin for under keel clearance in a choppy sea.

As the vessel approached the East Walker Dyke, the vessel 'Telamon' suffered a mechanical problem whilst entering the Trent, and
swung to proceed back out. The 'RMS Laar' was committed into the Trent and ran aground whilst avoiding the outbound vessel
'Telamon'. The pilot turned the vessel and stemmed the tide at West Walker Dyke. Once the outward vessel had cleared the trent,
there still appeared to be rising water at West Walker Dyke, so an attempt was made to enter the Trent. As the vessel approached the
Apex, it became apparent that the tide was ebbing. Once past the Apex, the pilot requested that the Captain contact his agent and try
to arrange a berth at Flixborough or Grove, or swing and proceed back out to anchor.
Whilst passing Cliff End, the vessel was set onto the bank, starboad helm was applied but the port bow caught the ebb tide and the
vessel moved across the river. The vessel grounded in the ness by island house at 0304. She refloated on the next tide and proceeded
to Blacktoft.

RMS Laar was in transit to Grove Wharfs in the Trent, running 1 mile astern of the vessel Telemon', who was also in transit to Grove,
berthing upriver of the RMS Laar. RMS Laar had passed WWD and was just at the Apex light committed to entering the Trent, when
the Telemon ahead indicated on VHF that she had a problem. It looked like the Telemon was swinging fully around to port, so the pilot
confirmed this and agreed a red to red passing. As the RMS Laar progressed further, it appeared that the Telemon was, if anything, to
the west of the leading lights, and not lying as expected. At this stage, it was felt that the only option was to pass close around the stern
of the Telemon. The vessel then passed close to the west of the Telemon, but then grounded just upstream of his position, on Island
Sand (Flats Light 120 x 0.25nm. Attempts were made to refloat, but the tide was pushing the vessel further onto Island Sand and at
0355 it was confirmed that the vessel would not be refloating that tide. The vessel ballasted to 4.2m E/K at this point. Tugs were
requested for PM tide on 21st, but due to problems at KGD, only Little Shuva was available from Goole. Attempts to refloat using Little
Shuva were not effective and vessels position and heading did not change at all. Tugs were requested again for AM tide on 22nd, and
Lashette and Gillian Knight attended. Attempts were made to work the RMS Laar astern, and a 70 degree change of heading was
achieved, but the vessel did not move astern. The tides up to this point did not follow predictions and were generally below throughout
the river. Tugs were requested again for PM tide on the 22nd. The Humber Charter attended with a relief pilot and then ran survey lines
around the vessel to confirm soundings. On this tide, indications were that the tide was above throughout the river and at 1511, the tide
was some 76cm above at Burton Stather. With the vessel on a drying height of 0.8m, she should have refloated at 1600. She came off
on her own power at 1540, without tug assistance. Guardsman was in attendance at 1545. The vessel was manoeuvred astern into
deeper water on the west side of the Trent and then handed the vessel over to pilot M. Taylor, who completed the passage to Grove
Wharfs (LM1). The Guardsman assisted

While trying to swing the vessel off Blacktoft, the ship drifted too far south and grounded. The initial plan was to swing to port but, as
the vessel was right handed with a dead slow speed of 5.5 knots, the starboard swing could not be corrected with the standard rudder.
When swinging to starboard, the vessel drifted too far south and hard to starboard with half ahead was ordered. The ship started
swinging to port after which the vessel touched the bottom. After approximately 20 minutes, the vessel refloated and swung to the west
of Blacktoft.

Departed lock. Difficult to get situational awareness owing to vessel design and very black night. Headed for East side lead. At one
stage checked the radar, speed 2.8 knots with helmsman struggling to steer. The vessel swung to port against the tide. Hard to
starboard helm was ordered but vessel stopped aground. Engines were operated astern with no effect. Engines were stopped until the
Little Shuva could arrive, as she was in Goole. Once tug arrived, the bow was pushed and vessel continued passage.

The vessel had the use of two tugs for her arrival at Grimsby. Whilst in the basin, the stern tug was pulling to slow the
vessel down. However, the ship was still going too fast in the northerly wind. Slow astern was given on the engine and
the stern came to port, making contact with the dolphin. In doing so, the cross bar on the dolphin detached, upon which
was the fixed red light.

deep) and split (approx 0.15m long x 0.03m wide) on the port shoulder. When exiting the lock, the vessel made contact
with the fendering on the east Inner side of the lock and one fender panel was displaced.

On arrival at KGD, the Master had the con and swung the vessel to the flood tide to approach the lock. On final
approach, the port shoulder landed heavily on the approach jetty. When the vessel lifted in the lock, the boatmen
reported damage to the shell plating. On further inspection by the Master, the shell plate was holed approx 30m abaft
the stem and 4m above the waterline. The hole was approximately 150mm diameter.

soften contact). There was no damage to the vessel but the Flixborough tide gauge was broken.

On the approach, vessel was setting towards West Jetty, so altered course to port to bring head to tide. Vessel then
made headway into the Bellmouth. Once inside, aft tug instructed to lift port quarter and forward tug asked to hold bow
in centre of Bellmouth. The starboard bow continued to fall to starboard and made contact with West jetty fenders,
causing damage. The vessel then proceeded into the lock and made contact with the east Outer lock gate, causing
damage to the walkway.

Entering Imm Lock on ebb tide, v/l stopped turning to port and gathering headway. As v/l would not turn used astern eng
but v/l contacted WJ causing small fender damage, ladder bent and concrete chipped. Fwd bulwark pushed in 15cm
on v/l.

starboard on approach. When approaching the lock, the speed was reduced and contact was made with the West Jetty
while swinging to port. As the bow was still swinging to port, the third fender was dislodged and the fourth fender
misplaced below the waterline. There was a small dent and scratch to the paint on the vessels stem. The vessel
continued her passage without further incident.

Pilot reports vessel landed heavily at the jetty whilst berthing causing some minor damage to a ladder on the installation.
No apparent damage to the ship, no injuries and no pollution. Berthing master is aware of damage. (Wind ENE 4-5).
Vessel on approach to IGT with Master on controls under guidance of pilot. On final approach, the Master started to
carry out his own manoeuvre and disregarded advice from the pilot. The vessel landed heavily on the fenders and made
contact with the ladder. Slight damage was done to the ladder on IGT jetty.

Vessel departing 8 Quay Imm has made contact with a Exxtor Terminal berth, with her port quarter, damaging a bollard
and some coping stones. There is no reported damage to the vessel. No injuries or pollution. ADM aware. Pilot awaiting
tug assistance.
Pilot boarded the vessel, no tug ordered, Wind E'ly 10 kts. Vessel was stbd side to on 8 quay Henderson's. The
headlines and sternlines were let go, and the stern began to drift to port. The pilot instructed hard to port and slow
ahead with little effect. The stern continued to drift to port and the pilot instructed to let go the fwd spring to settle
alongside Exxtor berth. When the port quarter landed on the Exxtor, the stern quarter overhung the quay and came into
contact with Bollard Number 5, breaking this as well as scuffing a 3m length of concrete fending and damaging a tyre
fender. A tug was ordered and assisted the vessel into the lock without further incident.
The Master had only joined the vessel 24 hours previously and was reluctant to handle the vessel.

0838 Vessel manoeuvring alongside HST5 with difficulty. Pilot and Master request Switzer Tug for immediate
assistance. Switzer Kathleen replies and contacts broker for instructions. 0842 Switzer Laura is mobilising to assist
from the Immingham Outer Harbour. 0856 Laura on station at HST. 0925 Vessel safely alongside at HST5. Pilot calls VTS
to report vessel made contact with the dolphin at Berth 2 whilst making her first approach to the berth. There has been
some wooden fendering damage, and some slight denting to the ship's hull (Reported by Laura). No one was injured and
there is no pollution.
Whilst backing down onto HST berth 5, a gust of wind (18 knots) caught the bow, causing it to drift off to port, coming
into contact with No 1/2 berth dolphin.
The incident occurred during an assessment for the Masters PEC. The Master was advised to order a tug if berthing on
berth 5, as the bow and stern thrusters are not powerful enough if caught in a gust of wind or if the tide on the wrong
bow(flood) or quarter (ebb). The fender which is replacing No 1 panel fender on Berth 2 was broken. There was a dent
and scratches to the port bow and rubbing band was torn.

caused damage to the catwalk. Possible dent on vessels bow.
Throughout the manoeuvre, the Master had the controls with advice from the pilot. Inside KGD, the vessel was
proceeding to berth at QE 9Qy. When abeam QE 8Qy, the speed was reduced to less than 1.5 knots and the spring was
run ashore. The pilot instructed the Captain to put the engines slow astern and the bow thruster to port, as the vessel
was right handed. Despite this, the W'ly wind caught the stern and pushed it to port, emphasized by the transverse
thrust. The bow swung to stbd and landed on the quay so pilot instructed hard port rudder with kicks ahead to lift the
stern. The port quarter made light contact with the westerly most catwalk. As the vessel moved ahead, the wind
pushed the stern onto the second westerly catwalk, bending it. At this point, tug assistance was requested and
pilot/master informed that this would take 20 minutes. The stern then landed on the third dolphin. The pilot requested that
the master make a headline fast and run another spring. A heaving line was then run from forward to aft so that a
stern line could be secured. With the headline, spring and engines movements, the stern was brought in and the vessel
secured to the quay. The bow thruster was running to port throughout the incident. The tug was cancelled. Damage
caused was to the catwalk between the 2nd and 3rd western dolphins which were bent. The berthing master informed
the pilot that the bow had made contact with the crane leg but the pilot is unsure when this occurred. Damage to the
vessel was; two dents to stbd bow bulwark, dent on bulbous bow stbd side, dent on port quarter bulwark.

V/l made contact with South Dock Bridge in Goole with slight damage to a number of stanchions.
No reported damage to vessel.
Whilst manoeuvring in Goole Docks, the Master was on the controls taking advice from the pilot. While passing through
South Dock Bridge, a spring was deployed and used from the port shoulder. On entering the bridge gap, the port
shoulder was alongside the bridge island with the stern tight, but not touching the dolphin. When easing ahead to clear
the dolphin, the overhang of the bow came into contact with the bridge hand rails. Approximately 2-4 sections of hand
rails were bent on the South Dock Bridge. The vessel then berthed without further incident. The pilot suggests that the
bridge hand rails be folded down to stop this type of incident occurring.

Pilot reports V/l made contact with a Bulk handling crane on 10 Quay.
Slight damage to railings on the vessel but no apparent damage to the crane.
The Master was manoeuvring the vessel in KGD with the wind SW 3-4. The wind gusted to SW 5/6 and the vessel
started closing onto 10 Quay knuckle and the vessel Isidor (on QE 10 Mid). The decision was taken to manoeuvre the
vessel ahead of the Isidor, land alongside and request a tug. Whilst manoeuvring alongside, the wind gusted again,
catching the stern. The vessel landed on the quay with the port quarter, making contact with crane. There was no
apparent damage to the crane but the railing on the ships monkey island were bent.

Landed heavily on approach to berth. Damage to roller fairlead on vessel's bow but no damage sighted to fendering at
IGT.
On approaching IGT using tugs, tug appeared to have difficulty moving into position and used more power than
requested. The result was v/l port shoulder touching fendering causing the protection bar around roller fairlead being
displaced and cracking weld at deck level.

Pilot reports contact with structure when berthing alongside Henderson Quay in Immingham Dock. There was a dent in
the hull plating 20cm x 30cm, approximately 2 cm depth.
Master and Pilot on bridge at the time of the incident. Master had the con during manoeuvres with tug fast centre lead
aft. Vessel left Immingham lock to proceed to Henderson Quay. As the vessel approached the quay, the wind was having
a big effect, setting the ship towards the quay. The set speed was too high, the Pilot advised the Master to use the tug to
counteract the set. This achieved the desired effect. When the ship was approximately 5m from the quay, the Master
asked the tug to proceed to the port quarter. The vessel made contact with the quay just forward of the accommodation
with 25 metres still to come ahead to be in position. The impact caused a dent in the hull plating 20cm x 30cm,
approximately 2cm depth. The berthing master advised that the damage had been done as the point had landing had
been on one of the protruding steel ladders on the quayside. In the opinion of the pilot and the master, the berth was not
correctly set up for the size of the vessel, with the 2 fenders set in the incorrect position.

Whilst transiting South Dock Bridge inward, the bow flare touched and rested on railings, bending them in. The Master
was on the controls taking advice from the pilot. The forward spring was in place but not used, as the bow was too far
through the bridge entrance to be used effectively. Crew had not sent forward spring when instructed. 5 railings were
bent on the South Dock Bridge walkway, no reported damage to the vessel.

Vessel moving from Imm Dock to the Imm Outer Harbour. Captain calls VTS to inform us that on leaving the lock the
vessel's starboard shoulder came into contact with the lock causing some minor damage to the hull. (Small dent and
10cm split in plating). No one was hurt and there is no pollution. Vessel is at this time all fast in the Outer Harbour. ADM
at Imm reports no damage to the lock or fendering etc.
Whilst exiting Immingham Lock, the vessels starboard shoulder came into contact with the lock side, with ships speed
approximately 0.75 Knots.
Lockhead reported to vessel that contact had been made, the vessel lifted off and proceeded to the Immingham Outer
Harbour. On inspection, there was an 8" vertical gash and 4cm indent for approximately 3m on the starboard shoulder
shell plating. No damage reported ashore and there was no pollution.

Pilot reports vessel making contact with the Dock Wall whilst swinging in Albert Dock.
Vessel swinging to starboard in turning area at 0610. The fwd station reported swinging clear by 20m with engine
moving astern. The vessels bow made slight contact with the Dock Wall. The crew reported no damage forward.
However, the fencing was damaged at HOTA.

Whilst berthing in Goole vessel made contact with the South Pier causing minor indentation to the stb'd shoulder. No
reported damage to the dock.
Vessel was backing down towards Ocean Lock on arrival at Goole. No problems were encountered until off South pier
when the bow took a sheer to starboard. Full bow thrust to port was applied and speed increased to slow astern
(vessels engine was stopped and drifting with the tide at the time). Contact was made with the end of South Pier. There
was an indentation approximately 1.0m x 0.7m x 0.1m just abaft the starboard bow.

Whilst departing KGD and approaching the lock the vessel made contact with the inner west knuckle causing damage to
the vessel in way of the stb'd bow. A breach of the hull of approx. 3 metres was sustained which is about 2metres
above the waterline.
The vessel then sheered to port causing the port bow to make contact with the inner east knuckle causing timber
damage to the lock.
Vessel returning to a berth to ascertain damage and effect repairs.
Pilot report to follow.

lock, with a strong tide as the gates open. Once bow was in the lock, the vessel set to port and landed on the fender,
breaking the piece of timber above the fender wheel. No damage to vessel.

On approach to the vessels berth (IOT1), the vessel sheared to starboard resulting in the port quarter closing quickly
towards the berth. The after tug, Svitzer Valliant, was switched from port to starboard quarter and the forward tug
increased push. In hindsight, the pilot overcompensated for the starboard shear and the vessel swung to port, resulting
in the port shoulder landing heavily on the forward fender.
There was no damage to own vessel, however, ashore, the forward fender handrail was bent and a bolt torn from the
rubber shock absorber.
02 Aug HOLSTENTOR

Pilot calls to report that whilst manoeuvring through South Dock Bridge for Caldaire Terminal the vessel made contact
with and subsequently bent a railing stanchion. Pilot confirms the vessel did have a spring line out at the time. There
was no damage to vessel, no one hurt and no pollution.
On the bridge at the time of the incident were the Master in the centre of the wheelhouse on the controls and the pilot on
the port side enclosed bridge wing. The vessel was transiting Goole Docks from ocean Lock to Caldaire terminal.
Vessel entered South Dock Bridge with fwd spring out. As soon as the aft crew reported that the stern was clear of
the pile protecting the steel terminal, the spring was checked. The rudder was hard to port with the engine on minimum
ahead. The bridge man reported vessel closing on bridge, spring held, thruster two steps to port, engine stopped. Vessel
contacted bridge railings and bent one stanchion. No further damage was reported and the vessel berthed at Caldaire
without further incident. There was minor damage to the vessels paint work only.

On departing New Holland Dock vessel suffers engine failure, locking telegraph astern causing vessel to contact with
New Holland Pier. Awaiting New Holland staff to inspect pier.
2207 - Pilot informs VTS vessel has full manoeuvrability and is proceeding to Holme Hook anchorage for bunkers, no
initial reports of damage, vessel will carry out full assessment in anchorage.
Duty Manager informed, MCA Informed. MCA will inform MAIB.
2345 Call from New Holland Shipping informs VTS of serious damage to the jetty but a full inspection to be
carried out during daylight hours.
V/l to proceed to Bull anchorage after bunkers.
0008 Bunker barge Rix Phoenix and Pilot declare extensive damage to vessel's stern. Master is still happy to sail as
damage is 2m clear of waterline but is talking to owners and class.
0025 Duty Manger and HM updated. HM wishes vessel to anchor in Hawke Anchorage till Owners, Flag State and Class confirm
vessel is seaworthy. Vessel declares it is fit to bunker, with no damage near to any fuel tanks. MCA updated.
For departure, the Master and Pilot were on the bridge. The vessel was trimmed well by the stern which limited visibility,
so control was taken on the port side. The position of the controls was such that in order to gain a view of the ships
side, the pilot needed to stand at the control station, so he agreed with the Master that the pilot would operate the
controls and give a running commentary so he could verify what was being done. The vessel left the dock at 2200. The
vessel left under control and , as the bow cleared the knuckle, a controlled turn to port was started.
As the after visibility was limited, the pilot moved to the starboard side, leaving the Master on the controls - enabling the
pilot to get a better view of the pier. The Master did not understand the request of the pilot and moved control to the
starboard side. Prior to transfer, the engine was set to O position, rudder amidships and bow thruster stopped. Once
control had been moved, the display panel was showing an engine setting of full astern, despite the telegraph being set
to zero. As the ship picked up sternway, the pilot instructed the Master to take control on the port side again and give
full ahead. However, whilst this was happening, the vessel had picked up considerable sternway and made heavy
contact with New Holland Pier. After the collision, the crew were accounted for, control regained and VTS informed.
There was damage to the concrete structure, handrails and conveyor belt cowling on New Holland Pier. There was a
deformation of the steelwork on the vessel on the starboard quarter, extending 10m. Forward down starboard side,
there were 2 spits in the steelwork on main deck, starboard quarter.

The vessels port bow plating touched the west inner lock gates which were in the housed position. The vessel was
entering the lock using the Main Engine, rudder and bow thruster. A spring line from the starboard bow was
subsequently used t bend round the knuckle and into the lock.
The Berthing Master on the lock reported that there was no damage to the lock gates, but it would be checked again
later by the engineering staff.
No damage to the vessel.
11 Aug VERITIAS H

Pilot reports that vessel made contact with the west knuckle on 10 Quay, whilst manoeuvring in KGD. No reported
damage.
The Master (Tripping Number 191), 2nd Mate and pilot were on the bridge. The Master conducted his own manoeuvring
throughout.
The vessel had left QE 9 and was proceeding stern first to the square for lock entry. Control of the vessel was with the
Master on the port bridge wing controls. It was apparent as the vessel proceeded that we would not clear the knuckle
on 10 quay. As the vessel approached the knuckle, the Master lifted the stern to starboard, which stopped the stern
closing momentarily, but did not lift it away.
At this point, the pilot suggested another kick ahead with port helm, but astern power was already being applied. The
stern dropped off quickly to port again and contacted the knuckle. The pilot reported this to KGD as the vessel continued
into the lock. The vessel proceeded without further incident and checks were made once in the lock, internally and
externally, for damage. None was evident.
21 Aug STAR CURACAO
KGD
On approach to KGD, the vessels bow made contact with the Western Approach Jetty.
Whilst making the approach to enter KGD, the Master asked or a distance from the Second Mate forward. The distance
given was 20 metres. This distance was incorrect and, although the vessel was coming around to starboard, the bow
made contact with the Western Approach Jetty. On the bridge at the time of the incident was the Pilot, master and Chief
Officer.
The pilot was informed by the Dock Mater that there was damage to 2 upright timbers and 2 horizontal timbers and
buckling of a plate on the deck. There was slight indent on the vessels flare and damage to paintwork.
23 Aug COLLINGHAM

Master of Collingham reports that whilst a crane was recovering the narrow boat 'Moorhen', secured alongside the
Collingham, the top of the crane came into contact with an overhead power cable, causing it to break.
On recovery of narrow boat from the top side of Gunthorpe, the Collingham began to proceed upriver to West
Stockwith Lock, with the narrow boat secured starboard side alongside. The crane holding the narrow boat clear of the
waterline was jibed up. In between Gunthorpe and West Stockwith Lock, overhead power cables were observed.
Unsure of the height of the power cables, the work continued. Once directly under the cables and unable to lower the
crane because of the narrow boat being supported, the crane missed the first power cable but came into contact with
the second.
This caused the power cable to break and it fell into fields on both sides of the river.
28 Aug BIRKA CARRIER

Reports making contact with west outer gate whilst entering lock inwards. Damage to stanchions on lock gates.
Master and Pilot were on the bridge at the time of the incident. As the vessel entered the Immingham Lock, the port
shoulder made light contact with the West Outer Lock gate causing six of the handrail stanchions to bend inwards at the
base (hinge), and one chain to break. No apparent damage to the vessel.

Vessel inward from sea to Imm Dock. Pilot reports making heavy contact with the Eastern Approach Jetty causing
damage to the ship's bow above the waterline. No injuries or pollution reported. Immingham report loss of one fender
and damage to two others. Awaiting further information from Dock. 0920 From Imm one fender has been sunk, with two
others damaged. Warning notice sent to Data Centre for pilots information regarding fender damage.
On boarding the vessel at the Spurn Light Float, the pilot car was sighted and no defects confirmed by the Master. It
was agreed that the pilot would conduct the transit up the river and the Master would manoeuvre the vessel into the
lock. The Chief Officer was to assist in communications.
On passing the A1 dolphin, the Master took over the manoeuvring, the plan being to head the vessel into the wind and
allow the tide to set the vessel into a position where the ship could be lined up for the lock. The Master commenced this
manoeuvre from the center console.
As the vessel passed inside the line of the IOT, the pilot noticed the speed was 5.6 knots and increasing. The pilot
challenged the Master which was acknowledged. As the vessel approached the East Jetty, the Pilot challenged the
Master again regarding the distance as it looked very close. Radio contact was made with the officer fwd for the first
time with a reply received in Icelandic. The pilot called Immingham on Ch 68to ask for distances, with the reply 35m and
closing fast. The vessel then started to turn to port, possibly due to the tide acting on the port quarter as the bow came
out of the tide. With the vessel still approaching the jetty fast, the pilot suggested that the engines be put aster. There
was then a lot of radio communications, but again all in Icelandic. The engine was then put astern and the bow thruster
full to starboard, all from the centre console.
The Master then went to the port wing console to take control, but this must have failed as he came back to the centre
console. The collision happened at this point. During the incident, all communications were conducted in Icelandic.
The vessel was holed on the bow above the waterline and dents in the same area.
Ashore, three fenders were damaged, one was missing and two were still attached to the approach jetty.
15 Sep WINDSTAR

On departure from N H Dock v/l clears the dock and lands alongside New Holland Jetty, at present v/l appears to be
held alongside the Jetty with the flood tide.
Albert Dock HW 2053/15th tide running late.
Albert Dock peaked at 2110/15th.
2120 Windstar clears the Jetty and resumes outward passage.
Pilot reports no apparent damage to Jetty or Vessel.
Master, Pilot and Chief Officer on the bridge. The Master did his own manoeuvring with advice from the pilot.
Tidal height on exit, 6.95m.
The vessel moved astern in the dock and repositioned. The spring was sprung off to wide angle, then started to move
astern out of lock. The pilot advised half astern as stern started to clear, then full astern. Tide was noted to be stronger
than anticipated. While checking on the starboard quarter, the pilot noticed that the Master had stopped the engine and
the stbd swing had ceased. With wind and tide pressing, and bow thruster not working, it was quickly obvious that the
vessel would not clear the dock approach area. The vessel made an orderly landing on the access jetty, waited for the
tide to turn , then proceeded without incident.
No damage reported to the vessel or jetty.

On entereing KGD lock, the vessel scraped the lock wall, damaging a steel plate.
During the transit through the lock, the port bow of Hold No. 1 scraped the lock wall, damaging the steel plate.

On departure from Albert Dock the lock head man informed the Pilot that the vessel had made contact with the North
Inner lock gate causing damage to two stanchions. No reported damage to the vessel.
Master and Pilot on the bridge with the Master controlling the vessel from the starboard bridge wing controls, pilot
advising.
Vessel stbd side to SE corner. Commenced letting go aft holding the forward spring. Attempted to bring stern out with
engine, but vessel was very slow. At 0040, let go spring and thrusted bow round to lock, then came ahead and entered
the lock.
The vessel was very slow turning into the lock, even though 300KW bow thruster full to starboard, helm hard to
starboard and engine ahead. Landed starboard shoulder on knuckle and vessel slowly came around.
The ADM at Albert Dock reported that the vessel had bent 2 stanchions on the north side inner gate. Vessel crew on
fwd station stated they did not see the vessel make contact.

The Dock Master confirmed that there was no damage to the jetty structure.
There was a scatch on the port bow of the vessel. The Master believes that there was possible contact with the
bulbous bow, but unable to confirm as below waterline.

Pilot reports vessel made light contact with a crane walkway whilst manoeuvring alongside 1 Quay in Immingham Dock.
There is no apparent damage to the vessel or crane, no one was injured and there was no pollution.
Whilst manoeuvring alongside No. 1 quay Immingham using two tugs, the vessel landed not quite parallel, stern first, with
the bow 2m off.
The support of No 3 crane on the vessel made contact with the railings of the shore side crane on No 1 quay. The speed
of the vessel was very slow. The railings on the crane showed no damage. There was a slight scratch on the crane
support on the vessel.
It was reported to Immingham that the vessel was moored dangerously close to the cranes as the ship was to use her
own cranes to load cargo. Any list caused by loading cargo may result in the vessel making contact with the cranes.
25 Nov WAALDIJK

0134 Whilst making an approach to the lock at King George vessel made contact with the wall causing paint damage to
the ships stb'd shoulder.
Master, Chief Officer and Pilot on the bridge.
The vessel swung off RT1 and waited for the 'Star Aruba' to sail - vessel headed into the wind.
The vessel's accommodation block had an overriding effect over the weak flood tide. Proceeded to lock at 2 knots over
the ground, allowing for strong wind effect. Contact was made with the lock entrance wall whilst unable to turn bow,
using full port rudder and full thrust. The vessel appeared to have a strong right had effect, which was made more
apparent when berthing on 10 Quay QE dock.
There was a slight indent (1.5m approx) on a high horizontal rubbing band on the starboard shoulder.
No apparent damage to the lock wall at KGD entrance.
03 Dec RIG
New H Pier
Pilot reports contact being made with N Holland Jetty, west outer, during berthing operation. Wind SW6/7
Master and Pilot were on the bridge at all times.
Vessel made standard approach to New Holland Pier West Outer for a starboard side to berthing.
When bow was close to jetty, spring line was sent ashore and the vessel stopped. The crew were instructed to make
spring fast on bits once the bow was landed on the fendering.
It was then attempted to bring the stern alongside, using 100% bow thrust to port, rudder hard to port with the engine
slow ahead.
Shortly afterwards, the crew reported that the bow had made contact with the structure of the jetty. The engine was
put half astern and the vessel cleared the jetty - all lines were let go. Vessel moved astern and prepared for another
approach.
Berthing was very difficult owing to weather and tide conditions.
04 Dec IVAN 2

Contact made whilst manoeuvring in the lock during outward passage from Grimsby.
Inga (Tug), not effective enough to reduce vessels speed without assistance from vessel engines. When approaching
Grimsby lock outbound, the tug Inga was required to reduce the vessels speed, as Dead Slow/Slow ahead was needed
to maintain steerage. As the tugs power was insufficient, engines were stopped when entering the lock. As a result,
the wind pushed the vessel out of line.
The outer lock gates were closed at this time. During the manoeuvre from berth to lock, communications with Inga were
poor, requiring 3-4 calls before a reply was received.
No damage was done to vessel. There was damage to a small piece of concrete on west side of the lock, and the
wood was cracked on the east inner roller.

Vessel made contact with shore crane whilst berthing 2 Quay Immingham Dock.
Whilst positioning the vessel alongside No 2 Quay, the port quarter had to be lifted to avoid coming into contact with the
bollard on No 2 Quay. This caused the port bow to close onto the crane despite the bow thruster pushing to starboard.
VHF confusion between the berthing master, pilot and tug aided and abetted the situation, and would have been avoided
if VHF jam hadn't occurred.

Pilot reports v/l outbound has caused possible damage to the East inner Walkway and the West inner Walkway, no
reported damage to vessel.
Pilot also reports vessel inbound caused damage to the East Outer fender.
Vessel was departing Grimsby, passing through the lock on the level. Bridge team consisted of Master, Pilot, OOW and
helmsman. Prior to departure from No 18 berth, the pilot was advised the vessel had no operational bow thruster or
bridge wing controls. He was also advised that damage had been sustained to the East Outer lock wheel and that
contact should be avoided. Two tugs were used, ' Este' forward and 'Inge' aft. Transit from the berth to the lock was
completed with Master and pilot conning from the port bridge wing. Whilst lining up for the lock, the pilot advised the
Master that he wished to remain on the bridge wing to observe the distance off the port quarter and the west wall of
Royal Dock. As the bow made final approach to the lock entrance, Este was instructed to enter the lock. At this time, the
vessel started swinging to port (Wind W'ly Force 5, on the port beam. In an attempt to counteract the swing using the
tugs, excessive headway was gained and at this stage the Master had transferred to the starboard bridge wing.
The engines were put half astern and the tugs stopped to regain control. At this point, damage may have been caused
to the West Inner gate rails. The vessel was very left handed. During the resulting passage through the lock, in an
attempt to keep the starboard side free from the East Outer rolling wheel, the after starboard section trailed along the
East Inner gate. The after section of the vessel is sharply cut away in construction.
Damage was also reported to the East inner lock gate walkway.

Contact made with bridge stanchions when passing through Goole south dock bridge.
Vessels position was difficult to control with engines, rudders and bow thruster. Captain was operating engine and
bow thruster. Chief Officer was operating steering gear. Due to difficulty in manoeuvring of the vessel, it was
necessary to land the starboard quarter on the wall, on bridge side. The starboard quarter bent the bridge stanchions.

POM calls VTS to advise of situation in IMM Dock. From Imm ADM the Sofia whilst manoeuvring in the dock for her
berth, had her engines jam astern and despite dropping her anchor has made contact with the Svitzer Ferriby.
The tug has suffered some fendering damage and the Saffier has an indentation and a 10 inch spilt to her stbd
quarter. No one was injured and no pollution was suffered. ADM has called DDM. VTS updates Duty Manager.
(Wind SW'ly 10-15kts).
1057 Pilot calls VTS to update us on the situation. Master had con throughout dock
movements, despite Pilot's advice, and both anchors were dropped in an effort to stop the vessel when the pitch
control jammed.
Vessel was inward to Immingham Dock and the vessel conducted the manoeuvre into the lock. At approximately
0820, the vessel left the lock to berth on Number 3 quay. Owing to the vessel being very right handed, the pilot
suggested that the vessel berth port side to, however, the Master declined and prepared to berth starboard side
to. At approximately 0836, the vessel slowed approaching the berth. The pilot was relaying distances from the
Berthing Master to the Captain and checking the distance off the starboard quarter from the vessel 'Clara'. The
Master set the port engine to 80% astern to gain some clearance forward. The Main Engine froze in the astern
position and the vessel gained sternway. The pilot ordered both anchors to be dropped and the main engine
emergency stop to be operated. At approximately 0852, the vessel made contact with the tug Svitzer Ferriby,
alongside No 5 quay. At 0856, the tug Svitzer Valliant was in attendance. At 0904, Main Engine power was
restored and the vessel proceed to berth on No 3 quay. Damage caused to the fendering on the starboard side
of the Svitzer Ferriby. There was damage to the shell plate on the starboard quarter of Saffier. The plate was
cracked, buckled and holed into the aft peak ballast tank.

Berthing at LE1 Grove when rudder stuck hard to starboard and vessel contacted with berthed vessel. Minor
damage to both vessels. No injuries. No pollution. POM advised. MCA advised by POM.
The vessel was proceeding to Grove LE1 with the master on the controls and the pilot conning. Having
completed a starboard swing off Scotch crane, the vessel proceeded across the river and was using starboard
helm to keep the flood tide fine on the port bow. The vessel was closing Grove Wharf abeam of MV Astra on
dead slow ahead. Port helm was applied and power increased to parallel the berth. The ships head continued to
starboard and the pilot queried the rudder position with the master, who replied that it had jammed with starboard
helm applied. This was not apparent immediately as the deck head rudder indicators were not working. When the
pilot realised that contact with the other vessel was unavoidable, action was taken to limit the damage by
coming astern on the engines. When the vessel was safely alongside her berth, steering was checked and the
rudder was working but jammed again to starboard during repeat tests. Damage to Linda Marijke was stbd side,
midships, ships side railings set in for a length of 11 metres. Damage to Astra was paint taken off rubbing strake,
port shoulder, deck level for a length of 2 metres.

During berthing at Grove LM1 bow of vessel grazed stern of vessel berthed ahead. No structural damage,
paintwork only to both vessels. No pollution. Minor damage to jetty.
The Captain was making a good approach to the jetty but when he went astern to take way off, the bow swung
to starboard towards the jetty. He gave port helm and a kick ahead to counteract this which swung the stern
towards the Fri Lake. The Master realised this and gave starboard helm to reduce the impact. There was
scraped paint on port bow of Fri Lake and scuffed concrete on Grove Wharf.
On own vessel, there was scraped paint on the starboard bow and the starboard quarter.

During manoeuvring off the IOT, the tug Svitzer Kathleen made contact with the vessel and caused damage to
paintwork on the ships side. The tug was proceeding in to let go her line when the contact was made.
On completion of the vessel swinging to port off IOT 3, the tug Svitzer Katheen proceeded in toward the vessel
to release her line. As she did so, the tug made contact with the vessel, causing damage to the paintwork on the
ships side. There also appeared to be some damage to the tugs crane.

Conservancy Activities
December 2010 to December 2011
During this twelve-month period, the Survey Section of Humber Estuary Services undertook 691
individual hydrographic surveys, including 3 contract multibeam surveys for outside parties, 86
for UK Dredging and 83 Ouse leading line surveys. The greater part of the data obtained was
passed to the United Kingdom Hydrographic Office to form the basis of Admiralty charts for the
Humber Area. All information gained from these surveys was processed and charted, resulting
in the production and distribution of over, 10,000 chart copies, mainly to be used in Pilotage and
general navigation, a number of the local charts also being offered for general sale.

The survey work resulted in the planning and carrying out of some 33 movements of floating
navigation marks, in order to mark the fluctuations in the alignment of the navigable channel,
mainly in the Upper Humber. In addition, out of a total of 108 local Notices to Mariners issued
during the period, based on survey results, 52 contained warnings of shoal water.
The monitoring and maintenance of navigational aids dealt with 12 reports of deficiencies in
floating marks and 6 in shore marks all being satisfactorily dealt with.

Final installation of a new multibeam system was completed in September and 3 outside
contracts surveys with the new system commenced.
