Investigation Consent Form

permission to the presiding paranormal investigative unit to conduct an investigation at the

Permission is herein granted to _________________________________ to conduct an investigation of
the property detailed above. Permission is also granted to conduct historical research of said
property, including, but not limited to, the review of historical documents, deeds, newspaper
articles, and other printed material, as well as interviews of previous owners/occupants,
neighbors, and eyewitnesses. The investigation process has been fully explained to me. The
presiding paranormal investigative unit releases the owner of the location from any liability for
injuries to investigators or damage to equipment owned by investigators and what may occur
during the course of the investigation. The presiding paranormal investigative unit, personnel, or
affiliates is not responsible for any damages to the property or persons before, after or during
investigation (waiver form). This excludes damages caused by individuals not affiliated with the
presiding paranormal investigative unit.
Confidentiality Agreement (Separate form): The presiding paranormal investigative unit assumes the
responsibility for the privacy of information obtained during the investigation. Information that is not
relevant to the paranormal investigation and public education in the paranormal research investigation
concerning this property, the investigations, witnesses, individuals interviewed in the course of the
investigation, or owners/occupants will not be shared to the public in any type of media. Information
obtained from interviews may be protected under a separate confidentiality agreement. Portions of
information or identities may not be divulged to the property owner/responsible party by the request of an
individual or entity interviewed during the course of the investigation. The agreement made herein can
only be altered or changed by written agreement between the property owner/responsible party and the
investigative team. This agreement is binding and applies to all past, present, and future members of
group members conducting the investigation.
Access to the property via this release is effective from today’s date upon approval. The
confidentiality agreement is effective for the lifetime of the property and all individuals specified
herein (Team members both past and current, property owners/occupants, witnesses, and all
individuals interviewed).
