The directory also includes useful information and tools related to emergency preparedness, funded in part by a
grant from the NJ Department of Health. We encourage you to share these materials with your co-workers, clients,

The Association is governed by a 16 member Board of Directors who is elected by our membership. The operations
of the Association are guided by the Executive Committee, Finance Committee, and the Board of Directors. The
Association provides a wide variety of opportunities for members to participate in the decision-making process of our
Association through committees and work groups which make recommendations to the Board.

The Home Care Foundation currently has a Grant from the NJ Department of Health & Senior Services, Office
of Public Health Infrastructure, Laboratories & Emergency Preparedness.
The Home Care Foundation of New Jersey has established and administers a Home Health Aide Scholarship
Fund from which scholarships are awarded to qualified Certified Home Health Aides who are pursuing a nursing
degree. The Foundation has awarded over 40 scholarships since its inception.

The Home Care & Hospice Association of NJ
Is the non-profit Association which represents and advocates for the full scope of home care and hospice agencies in
our state. The Association was founded in 1973 by a group of New Jersey home health agencies. Since that time it
has continued to grow, while still providing the same advocacy and education for which it was founded.
Our Mission is to serve as the catalyst for excellence in home care and hospice.
Our Goals are to inform, educate, assist and advocate for home care and hospice providers and the patients and
families they serve throughout New Jersey.
Our Members include licensed home health agencies, licensed health care service firms, licensed hospices, and organizations that offer services and products that support home care and hospice.
Our Services to members include:
Members only email network to keep home care and hospice providers well-informed about issues, regulations, legislation, emergency alerts, and new developments at both the state and national levels.

State and Federal lobbying representation advocating for the advancement of home care and hospice.
Discounted Educational seminars and an Annual Conference & Exhibition to bring the latest financial,
clinical, legislative, regulatory, and industry information to the administrators, managers and professionals
responsible for home care and hospice in NJ.
Participation on health care coalitions, committees and work groups with state and federal departments,
consumer groups, and other health care organizations to promote improvements in regulations, legislation,
outreach, and access to home care and hospice in New Jersey and the nation.
Promotion of new home care and hospice concepts and services through collaboration and special projects.
Development and distribution of the Home Care & Hospice Association of NJ Membership Directory
with detailed information about our member agencies, the communities they serve, and the services they make
available to consumers which are distributed to referral sources throughout New Jersey and surrounding areas.
Referrals as a result of the assistance we provide to consumers and professionals who have questions
about home care and hospice.
A website which provides information about home care and hospice to consumers and professionals, while
also serving as an instant communication tool for our members. The website also includes a calendar of
Association events, committee meetings, and educational programs.

HOSPICE IN NJ
Guide to Home Care & Hospice in NJ
Home Care and Hospice offers a broad range of health care and supportive services to people in their
place of residence. Based on the type of agency, services may include some or all of the following:

Professional nursing (RN and LPN)

Certified Home Health Aides (trained for medical and personal care)
Therapies: physical, occupational and speech-language
Medical social services and counseling
Homemaker and Companion Services (non-medical, no personal care)
Infusion therapy and specialized high-technology services
Laboratory and diagnostic services
Case management services
Nutritional support therapies and counseling
Behavioral and mental health counseling and support services
Preventative health and wellness programs
Hospice and bereavement services
Telehealth services
Licensure is a quality control which gives consumers the protection of state regulations and oversight.
There are three types of agencies in New Jersey that are licensed to provide direct hands on care to
consumers in their place of residence:

Home Health Agencies

Health Care Service Firms
Hospices
In addition to licensure, an agency may have one or more of the following:

Accreditation ACHC, CAHC, CHAP, NAHC, NIHCA and TJC. Accreditation will be required
for all health care service firms.
Medicare Certification required for care to be reimbursed by Medicare and for certain Medicaid
services.
Medicaid Approval required for home health aide and private duty nursing care to be reimbursed by
Medicaid.
NOTE In the state of NJ, Certified Home Health Aides (CHHAs) are permitted to work only for licensed
agencies and under the supervision of a registered nurse. They are NOT permitted to practice independently.
CHHAs receive training and certification and also must go through a criminal background check process. CHHAs
are re-certified every two years by the NJ Board of Nursing.

Through its Ready Campaign

The U.S. Department of Homeland Security educates and empowers Americans to take some simple steps to
prepare for and respond to potential emergencies, including natural disasters and terrorist attacks. Ready
asks individuals to do three key things: get an emergency supply kit, make a family emergency plan, and be
informed about the different types of emergency that could occur and their appropriate responses.
All Americans should have some basic supplies on hand in order to survive for at least three days if an
emergency occurs. Following is a listing of some basic items that every emergency supply kit should include.
However, it is important that individuals review this list and consider where they live and the unique needs of
their family in order to create an emergency supply kit that will meet these needs. Individuals should also
consider having at least two emergency supply kits, one full kit at home and smaller portable kits in their
workplace, vehicle or other places they spend time.
Recommended Items to Include in a

Basic Emergency Supply Kit:
Water, one gallon of water per person per day for at
least three days, for drinking and sanitation
Food, at least a three-day supply of non-perishable
food
Battery-powered or hand crank radio and a NOAA
Weather Radio with tone alert and extra batteries for
both
Flashlight and extra batteries
First aid kit
Whistle to signal for help
Dust mask, to help filter contaminated air and plastic
sheeting and duct tape to shelter-in-place
Moist towelettes, garbage bags and plastic ties for
personal sanitation

A CONSUMERS GUIDE TO PREPARING FOR A DISASTER

Creating a Disaster Plan
Meet with your family and discuss why you need to prepare for disaster. Explain the danger of fire, severe weather and
earthquakes to children. Plan to share responsibilities and work together as a team.
Discuss the types of disasters that are most likely to happen. Explain what to do in each case
Pick two places to meet:
1. Right outside your home in case of a sudden emergency, like a fire.
2. Outside your neighborhood in case you cant return home. Everyone must know the address and phone
number.
Ask an out-of-state friend to be your family contact. After a disaster, its often easier to call long distance. Other
family members should call this person and tell them where they are. Everyone must know your contacts phone
number.
Discuss what to do in an evacuation. Plan how to take care of your pets.
Find out where children will be sent if they are in school when an evacuation is announced.
Complete the Checklist
Post emergency telephone numbers by phones. (fire, police, ambulance, etc.)

Teach children how and when to call 911 or your local Emergency Medical Services number for emergency help.
Show each family member how and when to turn off the water, gas, and electricity at the main switches.
Check if you have adequate insurance coverage.
Teach each family member how to use the fire extinguisher (ABC type), and show them where it is kept.
Install smoke detectors on each level of your home, especially near bedrooms.
Conduct a home hazard hunt.
Stock emergency supplies and assemble a Disaster Supplies Kit.
Take a Red Cross first aid and CPR class.
Determine the best escape routes from your home. Find two ways out of each room.
Find the safe spots in your home for each type of disaster.
