Dear Sir,

Wish you a very good evening,I would like to draw your attention for the following
few lines mentioned below.

Today on 10.07.2012 (Tuesday) at about 6 pm my manager Mr.Tapas Kumar

Dhirsamanta told me that a debit note of Rs. 9.15 lacs is being asked to receive
from Mr.Mohanty a staff of your esteemed company( finanace
dept.),JSPL,NIsha,Angul.The reason of the said debit note of the said amount is
relating to the perry & docca charges levied on my company.

Let me tell you sir I have never received one perry or requisition to the person
concerned in writing( in charge of perry docca).The perry board were in the project
site much earlier before I got the work order & started the job in the month of Nov.
2011.It was Mr.Bhaskar Rao to whom you might have issued the perry board for
construction of the pump house.In Sept.2011 when I visited the site the entire pump
house was full of water & water level was 2.5 to 3 meters.Once I started a
dewatering of the pump house in the month of Nov. 2011. in the site Mr.J K sahoo &
myself found 20 to 30 pieces of perry were in concrete floor of the pump house.Mr J
K Sahoo has also told me that the perry is providing by us to do the job & you can
use it.10 to 15 sqm shuttering was also done but later on it was loose enough to
pour concrete this job was done by ECIC whose sub contractor was Mr.Bhaskar
Rao.During that time all the persons of your esteemed company who have visited
the site has told me we will provide you all support just you bring man power.At that
time nobody has told regrding the company policy about the use of perry board on
chargeable basis.

With all anguish & pain I would like to let you know that I have taken 480 pieces
water proofing ply(new) & adequate quantity buttom in the 1st week of
Nov.2011.The pump house was a left out job/balance job which I have done with a
complete assurance from the engineer in charge regarding full scale support to
complete the job.

Here 2 contractor has been deployed for the execution of construction of pump
house job ,I am the 3rd company to take up this job.The 2nd work order was for
construction of pump house was given to ECIC.ECIC has given it to a sub contractor
Mr.Bhasakar Rao .The actual work order of the pump house was issued to me on

18.11.2011,the gross value is Rs.45,36,395.00 but the 1st work order (before
amendment 18.11.2011 ) was issued to me on 14.11.2011 vide work order no 4561506189.

More over it is very piety to inform you in the month of Dec 2011 I am being
informed that JSPL will debit towards RMC supply & the rate was never known the
site engineer whose job is to inform the contractor regarding the
development/changes in policy/rate has failed miserably, the day when I started
working.

And subsequently it was communicated I will only get nearer to Rs.650 for laying
RMC per M3..Not only that by executing the entire pump house job I have till date
received Rs.14 to 15 lacs odd amount of money only. Where as my ply & buttom
cost Rs.8.75 lacs.The total labour payment till date Rs.5.50 lacs.my staff salary is
nearer to Rs.2.5 lacs.

Forget about the other materials including rod binding weire ,kanta, safety
materials & fooding expenses.So as a whole I have already spent more than Rs.19
lacs odd amount of money for pump house job

.Now you can understand where I am.Again I am being asked to be debited of

Rs.9.5 lacs.Now I am thinking as if it is the sky lab which is going to fall on my
roof.Honestly I am confessing before you,that I do not have the ability to meet that
debit note not only in the current year but also in the coming 3 to 4 years.

I am extremely unhappy when a contractor is executing the job after incurring

losses & again a debit note of such amount will pushing him to corner whee in his
life time he will not think up working with JSPL.where he has no fault the reasons I
have never requisitioned for nay perry or docca for construction of pump house.

So my submission before you

1. I have never requisition for a single piece of perry or docca neither verbally nor in
writing.

2.It was left out job where I have only received Rs.14 to 15 lcas odd amount of
money by incurring an expenses of nearer to Rs.20 lacs.
3.Nobody has told me regarding the perry docca charges to be levied on me if I
used it.
4.In every atosmoshphere may be good or bad(strike) I maintain utmost discipline
to carry out the assigned job.
5.Pump house has been constructed. But now unforeseen & unexpected financial
burden is twinkling in my door because job has been completed.

My prayer

1.Hence before your good office, expecting this sky lab(debit note) may kindly be
pushed in to the deep of pacific ocean instead of thinking to be dropped on my roof.
The reason is a poor soul may die without any reason - de- attire.

I have nothing to say but to pray your good office with all sublime &
humiliation.Please save one innocent heart without giving any extra jerk.
