underlying hypertension and gastritis. Patient presented to us with sudden onset of
epigastric pain for 1 day which lasted for about 20 minutes, dull in nature and non
radiating. The patient also complaint of shortness of breath, diaphoresis, orthopnea
but no PND. Patient claimed that he had no fever and and no bilateral lower limb
swelling.
1 year, with underlying hypertension and gastritis. Patient presented to us

On examination in ward, he was alert, conscious, not tachypnoeic, speak in

full sentence, hydration is fair, no pallor. Patient has mild pedal oedema on bilateral
lower limbs. Vital sign: BP: 128/94 mmHg, PR: 116bpm, RR: 20, Temp: 36.8 OC.
Respiratory examination: minimal bibasal crepitation at midzone and lower zone of
the lungs, Cardiovascular examination: dual rhythm no murmur. Per abdomen: soft,
non tender.
