CONTRACT OF SERVICE
KNOW ALL MEN BY THESE PRESENTS:
This contract of service is entered into by and between the DIEGO
H. PATIGAYON NATIONAL HIGH SCHOOL represented by MRS. VELLMA
LOU B. ALIMANZA, Secondary School Principal herein referred to
as the First Party and CLAUDINE G. BURGONIA of legal age, Filipino,
SINGLE with residence and postal address at Kapatagan, Kolambugan,
Lanao del Norte herein referred to as the Second Party;

WITNESSETH

That the First Party and the Second Party in connection of the
mutual agreement and covenant made herein by one to the other and by
these presents enter into the contract of service, whereby the First Party
hires the Second Party in the effective delivery of services, duties and
responsibilities of a WATCHMAN of DIEGO H. PATIGAYON NATIONAL HIGH
SCHOOL, which will be rendered and performed under the direct
supervision of the school principal subject to the following conditions:
1. That the Second Party agrees to give and furnish his undivided
time and services as a WATCHMAN of DIEGO H. PATIGAYON
NATIONAL HIGH SCHOOL, and perform such other functions
that maybe required of him by the First Party from time to time
effective January 1, 2015 to December 31, 2015 at the rate
of Three Thousand Pesos (Php 3,500.00) chargeable to Minimum
and other Operating Expenses and only payable every end of
the month under contract.
2. That the Second Party agrees to perform his duties and functions
anytime as required by the First Party with utmost diligence and
efficiency bearing always in mind the interest of service, and
shall not claim overtime pay in the execution thereof.
3. That the First Party and the Second Party both agree that this
contract of service will automatically terminate, cease and
expire in the last working hour of the last day of this contract as
herein provided.
4. That this contract maybe renewed or extended for another
period at the discretion of the First Party, when funds are
available and subject to good behavior shown by the Second
Party, with the ultimate provision that the First Party hereby
reserves the right to terminate the contract even before its

expiry date as herein indicated due to in availability of funds and

conduct unbecoming of a watchman as herein stated.
IN WITNESS WHEREOF, the parties affixed their signature this 1st
day of January 2015, at Diego H. Patigayon National High School,
Mukas, Kolambugan, Lanao del Norte, Mindanao, Philippines.

CONTRACT OF SERVICE
KNOW ALL MEN BY THESE PRESENTS:
This contract of service is entered into by and between the
LIANGAN NATIONAL HIGH SCHOOL, Liangan East, Bacolod, Lanao del
Norte represented by MRS. VELLMA LOU B. ALIMANZA, Secondary
School Principal herein referred to as the First Party;
-andMR. FELIX BESANA of legal age, Filipino, married with present
residence and postal address at Liangan, Bacolod, Lanao del Norte
hereinafter referred to as the Second Party;

WITNESSETH
That the First Party and the Second Party in connection of the
mutual agreement and covenant made herein by one to the other and by
these presents enter into the contract of service:
5. That the First Party needs the services of the Second Party as a
Contractor/Pakyaw of Labor and Materials of Sagayan Festival
Props, LIANGAN NATIONAL HIGH SCHOOL entry slated in July 2,
2014 at Mindanao Civic Center, Sagadan, Tubod, Lanao del
Norte.
6. That the Second Party is responsible for the construction of
Props for Liangan National High School contingent for Sagayan
Festival , and shall finish in time for the use during the practice
and during the contest as agreed;
7. That in view thereof, the First Party hereby hires the services of
the Second Party as Contractor/Pakyaw of the Props for
the Sagayan Festival 2014;

8. That the Second Party agrees to perform the duties and

responsibilities as contractor/pakyaw of the First Party with
outmost diligence and efficiency, and accomplish the specific
work under this responsibility with minimum direction from the
hiring agency;
9. That in view hereof, the Second Party is hereby compensated at
the amount of Fourteen Thousand Pesos only (Php
14,000.00) for labor and materials payable after the props will
be finish.
10. That it is also understood that this contract does not create
an employer-employee relationship between the First Party and
the Second Party, that the services rendered hereunder is not
considered and will not be accredited as government service,
and that the latter is not entitled to benefits enjoyed by the
regular personnel of the First Party;
11. That it is also understood that negligence or violation of the
above stated stipulations would mean a ground for misconduct
and Second Party will be considered black listed;

COMPUTER TECHNICIAN
2. That the Second Party will only perform and provide computer
services, repairs, and upgrades as requested by the First Party.
and the length of time required to service/repair the computers
is cant be predicted as agreed;
3. That in view thereof, the First Party hereby hires the services of
the Second Party in Contract
4. That the Second Party agrees to perform the duties and
responsibilities as COMPUTER TECHNICIAN of the First Party

with outmost diligence and efficiency, and accomplish the

specific work under this responsibility with minimum direction
from the hiring agency;
5. That in view hereof, the Second Party is hereby compensated at
the amount of Five Hundred Pesos (Php 500.00) for every
unit of computers being repaired;
6. That it is also understood that this contract does not create an
employer-employee relationship between the First Party and the
Second Party, that the services rendered hereunder is not
considered and will not be accredited as government service,
and that the latter is not entitled to benefits enjoyed by the
regular personnel of the First Party;
7. That it is also understood that negligence or violation of the
above stated stipulations would mean a ground for misconduct
and Second Party will be considered black listed;

1. That the First Party needs the services of the Second Party as a
PAINTER
2. That the Second Party will only perform the painting of the
School Name and School ID on the roof as requested by the
First
Party.
and the length of time required to service is cant be predicted
as agreed;
3. That in view thereof, the First Party hereby hires the services of
the Second Party in Contract
4. That the Second Party agrees to perform the duties and
responsibilities as PAINTER of the First Party with outmost
diligence and efficiency, and accomplish the specific work under
this responsibility with minimum direction from the hiring
agency;
5. That in view hereof, the Second Party is hereby compensated at
the amount of Two Thousand Pesos (Php 2,000.00) for
painting of the School Name and School ID on the roof;
6. That it is also understood that this contract does not create an
employer-employee relationship between the First Party and the
Second Party, that the services rendered hereunder is not
considered and will not be accredited as government service,
and that the latter is not entitled to benefits enjoyed by the
regular personnel of the First Party;
7. That it is also understood that negligence or violation of the
above stated stipulations would mean a ground for misconduct
and Second Party will be considered black listed;
IN WITNESS WHEREOF, the parties affixed their signature this 11th
day June 2016, at Diego H. Patigayon National High School, Mukas,
Kolambugan, Lanao del Norte, Mindanao, Philippines.

ELECTRICIAN
2. That the Second Party will only performing the installation of
the electric wirings in the Computer Room for the upcoming ICT
Packages of the DepEd Computerization Program (DCP) as
requested
by
the
First
Party.
and the length of time required to service is cant be predicted
as agreed;
3. That in view thereof, the First Party hereby hires the services of
the Second Party in Contract
4. That the Second Party agrees to perform the duties and
responsibilities as ELECTRICIAN of the First Party with outmost
diligence and efficiency, and accomplish the specific work under
this responsibility with minimum direction from the hiring
agency;
5. That in view hereof, the Second Party is hereby compensated at
the amount indicated in the Labor Rate charge/ Tariff charge by
the LANEA;
6. That it is also understood that this contract does not create an
employer-employee relationship between the First Party and the
Second Party, that the services rendered hereunder is not
considered and will not be accredited as government service,
and that the latter is not entitled to benefits enjoyed by the
regular personnel of the First Party;
7. That it is also understood that negligence or violation of the
above stated stipulations would mean a ground for misconduct
and Second Party will be considered black listed;
